摘要
目的研究比较宫腔镜辅助电切术与刮宫术对子宫内膜息肉不孕患者的疗效。方法选择2013年5月至2015年2月在中山市东凤人民医院接受治疗的110例子宫内膜息肉所致不孕患者进行研究,根据数字法随机分成电切组及刮宫组,每组55例。对比两组手术相关指标,随访1年,比较两组患者手术前后的月经量变化,两组患者的复发率及妊娠率,以及两组术后并发症情况。其中月经量的计算使用月经失血图法进行判断。结果两组的手术时间、术中出血量、以及术后康复时间相比差异未见统计学意义(P>0.05)。电切组在术后3个月、6个月及12个月的月经量均少于刮宫组,差异均有统计学意义(P均<0.05)。电切组的复发率显著低于刮宫组,而妊娠率显著高于刮宫组,差异均有统计学意义(P均<0.05)。两组对比术后并发症,差异均未见统计学意义(P>0.05)。结论宫腔镜辅助电切术治疗子宫内膜不孕患者,具有较好的疗效,且安全性较高,值得临床重视。
Objective To study and compare the effects of hysteroscopy electrocision and uterine curettage on patients with endometrial polyps infertility. Methods From May 2013 to February 2015,one hundred and ten cases of endometrial polyps infertility patients in our hospital were selected. According to digital method,they were randomly divided into electrocision group and curettage group with 55 cases in each group. The indicators related to surgery were compared,followed up for 1 years,the changes of menstruation before and after treatment,recurrence rates,pregnancy rates and postoperative complications of the two groups were compared. The menstrual quantity was calculated by menstrual blood loss chart method. Results There was no significant difference in the amount of bleeding during operation or operation time,postoperative recovery time between the two groups（ P〉0. 05）. The menstrual blood loss was less in electrocision group 3 months,6 months and 12 months after operation,and there were significant differences（ P all〈 0. 05）. The recurrence rate of electrocision group was lower than that of curettage group,and the pregnancy rate was higher than that of curettage group,and the differences were significant（ P all〈 0. 05）. There was no significant difference in postoperative complications between the two groups（ P〉0. 05）. Conclusion The hysteroscopic electric resection has good curative effect on endometrial infertility patients,and high safety,so it is worthy of clinical attention.
