超声弹性成像评分法与应变率比值法在鉴别诊断肝脏局灶性病变良恶性中的应用 被引量:9

摘要 目的:探讨超声弹性成像(UE)评分法与应变率比值(SR)法在鉴别诊断肝脏局灶性病变(FLL)良恶性中的应用价值。方法:对81例患者的89个FLL进行常规超声、UE评分法及SR法检查,并与临床诊断结果进行对比分析,绘制UE评分法及SR法鉴别诊断FLL良恶性的ROC曲线,计算二者的最佳临界点及ROC曲线下面积(AUC)。计算常规超声、UE评分法及SR法鉴别诊断FLL良恶性的敏感性、特异性、准确性,比较三种方法的准确性,P<0.05为差异有统计学意义。结果:1常规超声鉴别诊断FLL良恶性的敏感性、特异性、准确性分别为76.2%、72.3%、74.2%。2UE评分法鉴别诊断FLL良恶性的AUC为0.875,最佳临界点为2.5,其敏感性、特异性、准确性分别为90.5%、78.7%、84.3%,UE评分法鉴别诊断FLL良恶性的准确性高于常规超声,差异有统计学意义(P<0.05);3SR法鉴别诊断FLL良恶性的AUC为0.892,最佳临界点为2.71,其敏感性、特异性、准确性分别为83.3%、89.4%、86.5%,SR法鉴别诊断FLL良恶性的准确性高于常规超声,差异有统计学意义(P<0.05);4UE评分法与SR法鉴别诊断FLL良恶性的准确性差异无统计学意义(P>0.05)。结论:UE评分法与SR法鉴别诊断FLL良恶性的准确性均高于常规超声,为FLL良恶性的鉴别诊断提供了新的方法,二者在鉴别诊断FLL良恶性的准确性上无明显差异,具有较好的一致性。 Objective: To evaluate the value of ultrasound elastography (UE) scoring and strain ratio (SR) in differentiating benign and malignant focal liver lesions(FLL). Methods: Conventional ultrasound, ultrasound elastography scoring and SR were performed in 81 patients with 89 FLL, the results were compared with clinical diagnosis. Receiver operating charcteristic(ROC) curves of UE scoring and SR were portrayed, the cut-off points and the areas under the ROC curves (AUCs) were calculated. The sensitivity, specificity, accuracy of conventional ultrasound, UE scoring and SR were calculated, the accuracy of these three methods were evaluated. Results: ①The sensitivity, specificity, accuracy of conventional ultrasound in differentiating be- nign and malignant FLL was 76.2%, 72.3%, 74.2%. ②The AUC of UE scoring in differentiating benign and malignant FLL was 0.875, the cut-off point was 2.5, the sensitivity, specificity, accuracy of UE scoring was 90.5%, 78.7%, 84.3%. The accu- racy of UE scoring in differentiating benign and malignant FLL was higher than conventional ultrasound (P〈0.05). ③The AUC of SR in differentiating benign and malignant FLL was 0.892, the cut-off point was 2.71, the sensitivity, specificity, accuracy of SR was 83.3%, 89.4%, 86.5%. The accuracy of SR in differentiating benign and malignant FLL was higher than conven- tional ultrasound (P〈0.05). ④The accuracy of UE scoring and SR in differentiating benign and malignant FLL showed no sig- nificant difference (P〉0.05). Conclusions: The accuracy of UE scoring and SR in differentiating benign and malignant FLL arc higher than conventional ultrasound, they provide new ways to differentiate malignant from benign FLL, UE scoring is in good agreement with SR, there is no significant difference between UE scoring and SR.
