摘要
目的探讨腹腔镜经胆囊管切开联合钬激光治疗胆管结石的临床疗效。方法选取2013年1月—2015年4月景德镇市第二人民医院收治的胆管结石患者60例,随机分为观察组与对照组,各30例。对照组患者予以传统胆管手术及开腹手术治疗,观察组患者予以腹腔镜经胆囊管切开联合钬激光治疗。观察两组患者术中出血量、胃肠恢复时间、住院时间、住院费用、残余结石情况、复发情况及并发症发生情况。结果观察组患者术中出血量少于对照组,胃肠恢复时间、住院时间短于对照组,残余结石率、复发率、并发症发生率低于对照组,差异有统计学意义(P<0.05);两组患者住院费用比较,差异无统计学意义(P>0.05)。结论腹腔镜经胆囊管切开联合钬激光治疗胆管结石的临床疗效显著,可减少患者出血量,促进患者胃肠功能恢复,缩短患者住院时间,且并发症少,不易复发。
Objective To explore the effect of tlaparoscopic via the cystic duct incision and holmium laser in treating biliary calculi. Methods A total of 60 patients with biliary calculi were selected from, they were randomly divided into control group and observation group, 30 cases in each group. Control group were given traditional biliary operation and open surgery treatment, observation group were given laparoscopic via the cystic duct incision and holmium laser treatment. Intraoperative blood loss, gastrointestinal recovery time, hospitalization time, hospitalization expenses, residual stones, recurrence were ob- served and occurrence of complications between the two groups were compared. Results Intraoperative blood loss of observation group was less than that of control group, gastrointestinal recovery time, hospitalization time of observation group were shorter than those of control group, residual stones rate , recurrence rate, complication rate of observation group were lower than those of control group （ P 〈 0.05 ） ; hospitalization expenses showed no significant differences between the two groups （ P 〉 0. 05 ）. Conclusions Laparoscopic via the cystic duct incision and holmium laser have significant effect in＇the treatment of biliary calcu- li, can reduce the patients＇blood loss, promote the patients with gastrointestinal functional recovery, shorten their hospital stay, and fewer complications, has not easy to relapse.
