摘要
目的了解宝安区O型孕妇血清Ig G抗A（B）效价与新生儿ABO溶血之间的关系,为临床诊断及预防提供参考依据。方法采用国产微柱凝胶法对3038例孕16~40w的O型孕妇（其丈夫为A型、B型、AB型）作不规则抗体筛查和Ig G抗A（B）效价的测定,以1＋凝集强度为阳性判断标准,并以此血清稀释倍数的倒数为效价,并与临床确诊为ABO血型不合新生儿溶血病（ABO-HDN）209例产妇作相关性分析。结果 3038例O型孕妇Ig G抗A（B）的效价≥256为45.69%（1388/3038）,其中,夫妇A-O血型不合的O型孕妇为50.26%（681/1355）,B-O血型不合为41.94%（507/1209）,AB-O型不合的为42.19%（200/474）;209例临床确诊为新生儿ABO-HDN的O型产妇Ig G抗体效价≥256为98.56%（206/209）。结论国产微柱凝胶法检测夫妇血型不合的O孕妇血清Ig G抗体效价应设为〈256较为合理,对临床ABO-HDN的预测、诊断及防治有重要意义。
Objective：Understanding of baoan district O kang A pregnant women serum Ig G titer（B）the relationship between neonatal ABO hemolysis,provide reference basis for clinical diagnosis and prevention. Methods：Using domestic micro column gel method in 3038 cases of type O pregnant women in 16 ~ 40 weeks gestation（her husband for type A,type B,type AB）for irregular antibody screening and Ig G against A（B）the determination of potency,with 1 ＋ agglutination intensity is positive judgment standard,and the reciprocal of serum dilution ratio as the titer,and clinical diagnosis of ABO blood group of hemolytic disease of newborn（ABO HDN）,209 cases of maternal as A correlation analysis. Results：3038 cases of O type A（B）of pregnant women Ig G titer ≥256 was 45.69%（1388/3038）,among them,the couple A-O blood type is O type pregnant women was 50.26%（681/1355）,B-O type was 41.94%（507/1209）,AB-O was 42.19%（200/474）;Clinical diagnosis of 209 cases of neonatal ABO-HDN O maternal Ig G antibody titer ≥256 was 98.56%（206/209）. Conclusion：Domestic micro column gel method to detect the couple of blood type is O,pregnant women serum Ig G antibody titer should be set to256 is relatively reasonable,for clinical ABO HDN prediction,diagnosis and prevention is important.
