摘要
目的探讨超声早期诊断新生儿及3个月以内小婴儿发育性髋关节发育不良(DDH)的价值。方法采用5~12MHz高频线阵探头,对600例门诊疑似DDH的患儿髋关节进行筛查,同时与髋关节骨盆平片对比。结果600例疑似患儿行X线检查有147例髋关节存在不同程度的发育异常,其中完全脱位10例,半脱位15例,髋臼发育不良122例;X线诊断髋关节发育异常的147例患儿同时行超声检查,发现完全脱位10例,半脱位12例,髋臼发育不良117例,正常髋关节8例。两种方法诊断不吻合的3例半脱位、8例正常患儿1个月后随访,进一步行X线和超声检查被证实为半脱位和正常髋关节。结论与骨盆平片相比,超声能较好地显示髋关节软骨的解剖结构,早期诊断婴幼儿DDH,尤其是为股骨头骨化中心尚未出现、髋臼软骨X线检查不能明确的新生儿及3个月以内小婴儿DDH的早期诊断提供依据。
Objective To evaluate the application of ultrasound in early detection of developmental dysplasia of the hip (DDH) in neonates and infants less than three months old. Methods A total of 600 suspected child patients with DDH in outpatient department were referred for ultrasound screening using high frequency linear array probes (5 - 12 MHz). The obtained images were compared with X - ray photos of the hip. Results Among the suspected child patients were 147 with various developmental dysplasia in the hip by X -ray examination, including 10 patients with a full dislocation, 15 with subluxation, 122 with acetabular dysplasia. These patients were then referred for ultrasound screening, determining a full dislocation in 10 patients, subluxation in 12 patients, acetabular dysplasia in 117 patients and normal hip joint in 8 patients. One month later, subluxation and normal hip joint were determined by follow - up visits in 3 and 8 patients, respectively. Conclusion Comparing with pelvic X -ray, ultrasound screening can reflect the anatomical structure of the articular cartilage in the hip well, detecting suspect infant with DDH at an early stage. This technique is useful especially before the appearance of the femoral head ossification center when suspected neonates and infants less than three month old with DDP were difficult to be determined by X - ray examination.
