摘要 目的:探讨α-硫辛酸对早期糖尿病肾病(DN)患者的保护作用及其机制。方法:选取40例早期DN患者为治疗组,在常规治疗基础上加用α-硫辛酸(600mg/d),取同期查体健康者40例为对照组,治疗前及治疗6个月后检测患者空腹血糖(FBG)、糖化血红蛋白(HbA1c)、血肌酐(Scr)、尿微量蛋白(mAlb)、血清超氧化物歧化酶(SOD)、丙二醛(MDA)、肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、超敏-C反应蛋白(hs-CRP)等指标,进行对比分析。结果 :与对照组比较,治疗前DN患者的FBG、HbA1c、mAlb、MDA、TNF-α、IL-6、hs-CRP升高,SOD水平下降,差异具有统计学意义(P<0.01);采用α-硫辛酸治疗6个月后,DN患者的FGB、HbA1c、Scr较治疗前无明显变化,但mAlb、MDA、TNF-α、IL-6、hsCRP水平下降,SOD水平升高,差异具有统计学意义(P<0.01)。结论:α-硫辛酸对早期DN患者具有保护作用,推测其机制与抑制氧化应激及降低炎症水平有关。 Objective:To discuss the protective effect and mechanismof alpha-lipoic acid in patients with early diabetic nephropathy(DN).Methods:Selected 40 patients with early DN as treatment group,on the basis of routine therapy + alpha-lipoic acid(600mg/d),Select the same period on examination of 35 cases of healthy subjects as control group.Before and after 6months treatment detection in patients with fasting blood glucose(FBG),glycated hemoglobin(HbA1c),serum creatinine(Scr),microalbuminuria protein(mAlb),serum superoxide dismutase(SOD),malondialdehyde(MDA),tumor necrosis factor(TNF alpha),interleukin 6(IL-6),hypersensitive c-reactive protein(hs-CRP)index were analyzed.Results:Compared with control group,treatment of DN patients before FBG,HbA1 c,mAlb,MDA,TNF alpha,IL-6,hs-higher CRP present,SOD levels drop,the difference has statistical significance(P0.01).Compared with before treatment,after six months of alpha-lipoic acid treatment,patients with DN FGB,HbA1 c,Scr has no obvious change,mAlb,MDA,TNF alpha,IL-6,hs-CRP levels drop,SOD levels,statistically significant differences(P0.01).Conclusion:Alpha-lipoic acid plays a protective role in patients with early DN,speculated that the mechanism associated with inhibiting oxidative stress and reduce inflammation levels.
