摘要
目的评价QUATRIX(croma)非球面人工晶状体(intraocular lens,IOL)植入术后早期临床效果,对比研究与零球差非球面IOL植入术后的视功能差异,以期为非球面IOL的临床选择应用提供参考依据。方法收集年龄相关性白内障患者51例60眼,随机分为2组,试验组25例30眼,植入-0.22μm球差QUATRIX非球面IOL,对照组26例30眼,植入零球差akreos AO(Baush&Lomb)非球面IOL。术后3月,分别检查记录2组最佳矫正视力、5 mm瞳孔直径下对比敏感度及眩光对比敏感度,全眼球差及慧差、总高阶像差,并进行比较。结果术后3月,2组间最佳矫正视力无统计学差异。对比敏感度在12、18周/度空间频率上,差异有统计学意义(P<0.05),眩光对比敏感度在各空间频率无统计学差异。试验组全眼球差低于对照组,差异有统计学意义(P<0.01),慧差、总高阶像差2组间无统计学差异。结论 QUATRIX非球面IOL安全、植入方便,临床效果好,较零球差非球面IOL早期视觉质量好。如术前能明确患者角膜像差,个性化选择不同球差的非球面IOL,将更能有效提高白内障患者术后视觉质量。
Objective To evaluate the early clinical effects of QUATRIX( croma) aspherical intraocular lens( IOL)implanted in the patients with cataract,and to compare the differences in the effects,on visual function between QUATRIX IOL and zero spherical aberration aspherical IOL,so as to provide the reference basis for clinical application of aspherical IOLs. Methods Fifty-one cases( 60 eyes) of age-related cataract patients,were randomly divided into two groups. The test group,25 cases( 30 eyes),were implanted with- 0. 22 μm spherical aberration QUATRIX aspherical IOL,and the control group,26 cases( 30 eyes),were implanted with zero spherical aberration akreos AO( BaushLomb) aspherical IOL. Three months postoperatively,the best corrected visual acuity( BCVA),contrast sensitivity and glare contrast sensitivity with 5 mm pupil diameter,ocular spherical aberration,coma,total higher-order aberrations( HOAs) were detected and compared between the two groups. Results Three months postoperatively,there was no statistical difference in BVCA acuity between the two groups. There was significant difference in contrast sensitivity on 12,18 cycles / degree spatial frequency betweent two groups. There was no significant difference of glare contrast sensitivity in all spatial frequencies. All eye spherical aberration in the QUATRIX group was lower than that in the control group. There was no statistical difference in coma and HOAs between the two groups. Conclusions QUATRIX IOL is secure,easier to be implanted,and has good clinical effects. Compared with the zero spherical aberration aspherical IOL,QUATRIX IOL has better early visual quality. If we can determine the patients' preoperative corneal aberration,individual choice aspherical IOL with different spherical aberration will more effectively improve the postoperative visual quality of cataract patients.
