硫酸钙骨水泥椎体成形联合后路短节段椎弓根螺钉内固定治疗创伤性胸腰椎骨折 被引量:12

摘要 目的:探讨硫酸钙骨水泥(calcium sulfate cement,CSC)椎体成形联合后路短节段椎弓根螺钉内固定治疗创伤性胸腰椎骨折的临床疗效。方法:2012年6月至2014年9月收治50例胸腰椎骨折患者,男31例、女19例;年龄(39.42±16.53)岁;均为创伤引起,其中交通事故伤37例、坠落伤9例、压伤4例;爆裂型骨折26例、屈曲压缩型骨折17例、屈曲分离型骨折7例;T11骨折6例、T12骨折15例、L1骨折17例、L2骨折7例、L3骨折4例、L4骨折1例。经影像学检查,所有骨折椎体压缩>30%,椎管受压<30%或脊柱后凸Cobb’s角>20°,至少有一侧椎弓根完整。骨折至入院时间均≤7 d。均采用CSC椎体成形联合后路短节段椎弓根螺钉内固定治疗。比较手术前和术后3个月的脊柱后凸Cobb’s角、伤椎前缘压缩率、伤椎后缘压缩率、疼痛视觉模拟评分(visual analogue score,VAS)及神经功能改善情况,神经功能改善率=术后Frankel分级改善人数/术前神经功能障碍总人数(Frankel分级A、B、C、D级)×100%。结果:与术前相比,术后3个月时本组患者的脊柱后凸Cobb’s角、伤椎前缘压缩率、伤椎后缘压缩率及疼痛VAS评分均减小[21.12°±3.17°,2.79°±1.28°,t=37.910,P=0.000;(58.47±10.53)%,(5.68±2.54)%,t=34.460,P=0.000;(12.39±3.81)%,(2.67±1.92)%,t=16.110,P=0.000;(7.56±1.26)分,(2.19±0.68)分,t=26.520,P=0.000]。神经功能按Frankel分级,术前A级1例、B级2例、C级5例、D级8例、E级34例,术后3个月B级1例、C级1例、D级1例、E级47例,13例患者的Frankel脊髓损伤分级较术前改善,改善率81.25%。结论:CSC椎体成形联合后路短节段椎弓根螺钉内固定治疗创伤性胸腰椎骨折,可在短期内有效恢复椎体高度、纠正脊柱后凸畸形、减轻疼痛、促进神经功能恢复。
