阿奇霉素对大环内酯耐药支原体肺炎患儿的疗效及预后分析 被引量:2

摘要 目的:探讨阿奇霉素对大环内酯耐药(Macrolide-resistant,MR)支原体肺炎患儿的疗效并对其临床特点进行分析。方法:选取我院96例支原体肺炎患儿,对其咽部分泌物体外培养鉴定,并进行药敏试验鉴定出MR肺炎支原体(MR组),以大环内酯敏感(Macrolide-sensitive,MS)肺炎支原体为对照(MS组),两组均给予阿奇霉素治疗,观察记录临床症状特点,并测定相关指标,了解其疗效。结果:96例患儿咽分泌物行体外肺炎支原体培养,63例为阳性,该63例肺炎支原体进行体外药敏试验检测,耐药比例为63.5%(40/63);与MS组相比较,阿奇霉素治疗后,MR组患儿发热时间较长[(10±3)d,P<0.05];呼吸道症状持续时间较长[(16±4)d,P<0.05];C-反应蛋白(CRP)含量较高[(9.7±1.2)mg/L,P<0.05];在年龄和胸部X线片好转时间上,两组比较差异无统计学意义(P>0.05)。两组患儿均出现肺部炎症影像,MR组单侧肺病变24例(60.0%),双侧16例(40.0%),右侧肺病变率(14例,58.0%)高于左侧(10例,42.0%)。婴幼儿病变中出现斑片状阴影,1例出现少量胸腔积液。MS组单侧肺病变14例(60.9%),双侧9例(39.1%),右侧肺病变率(9例,64.0%)高于左侧(5例,36.0%),婴幼儿病变中出现斑片状阴影,无胸腔积液出现。MR组总有效率为82.5%,MS组总有效率为91.3%。结论:阿奇霉素对MR支原体肺炎具有治疗作用,但其对MS支原体肺炎疗效更好。 Objective: To evaluate the efficacy of azithromycin to macrolide-resistant( MR) mycoplasma pneumonia in children and to analyze the clinical characteristics. Methods: Ninety-six cases of mycoplasma pneumonia in children were investigated in our hospital.The pneumoniae mycoplasma from pharyngeal secretions were cultured and identified,the susceptibility testing in vitro was also performed. MR mycoplasma pneumonia was chosen as MR group,Macrolide-sensitive( MS) mycoplasma pneumonia was chosen as control group( MS group). Both groups were treated with azithromycin. Clinical symptoms were observed and some related indicators were measured to understand its efficacy. Results Sixty three children were positive among the culture of pneumoniae mycoplasma from pharyngeal secretions of 93 cases. The susceptibility testing in vitro showed that the drug resistance ratio of pneumoniae mycoplasma was63. 5%( 40 /63). After treatment,the fever time of MR group was( 10 ± 3) days,longer than MS group,the difference was statistically significant between the two groups( P〈0. 05). The duration of respiratory symptom was( 16 ± 4) days,longer than MS group,the difference was statistically significant between the two groups( P〈0. 05). The CRP content was( 9. 7 ± 1. 2) mg / L,higher than MS group,the difference was also statistically significant( P〈0. 05). There was no statistically significant difference between the two groups( P〉0. 05) on age and improvement time of chest imaging. The images of two groups were showed inflammation and lesions in lung. To MR group,24 cases in unilateral lung( 60. 0%),16 cases in bilateral lung( 40. 0%),and the lesions in right lung( 14 cases,35. 0%) was higher than the left( 10 cases,25. 0%).
