摘要
目的通过比较和分析梅毒螺旋体四种血清学检测方法的敏感性及特异性,寻找出梅毒螺旋体抗体合理的检测模式。方法应用甲苯胺红不加热血清试验(TRUST)、梅毒螺旋体酶联免疫吸附试验(TP-ELISA)、梅毒胶体金试验(SYP)及梅毒螺旋体抗体明胶颗粒凝集试验(TPPA)对我院183例临床确诊病例及217例健康体检者的血清标本进行检测,比较四种检测方法敏感性及特异性是否存在差异。结果 ELISA、SYP和TPPA三种方法的敏感性均很高,分别为96.7%、99.4%和98.9%,三者之间差异无统计学意义(P>0.05),ELISA、SYP和TPPA三种方法的敏感性均高于TRUST(P<0.05);四种方法的特异性均很高,分别为97.7%、98.6%、99.5%和98.6%,四者之间差异均无统计学意义(P>0.05)。结论结合各方法优缺点,探索出先用TP-ELISA初筛,阳性标本同时采用SYP比对,如两种定性方法结果不符,再用TPPA确诊,阳性标本最后用TRUST检测非特异性抗性滴度的合理检测模式。
Objective By comparing and analyzing the sensitivities and specificities of four kinds of syphilis serology detection methods, to find out a reasonable detection model of the Treponema pallidum antibody. Methods Serum samples from 183 patients diagnosed with syphilis and 217 healthy individuals in our hospital were tested by toluidine red unheated serum test(TRUST), Treponema pallidum enzyme-linked immunosorbent assay(TP-ELISA), syphilis colloidal gold test(SYP) and Treponema pallidum particle assay(TPPA). Then the sensitivities and specificities of the four methods were compared. Results The sensitivities of ELISA, SYP and TPPA were high, which were 96.7%, 99.4% and 98.9% respectively and had no significant difference(P〉0.05). The sensitivities of ELISA, SYP, TPPA were higher than that of TRUST(P〈0.05). The specificities of ELISA, SYP, TPPA and TRUST were 97.7%, 98.6%, 99.5% and 98.6% respectively, which had no significant difference(P〉0.05). Conclusion According to the advantages and disadvantages of the four methods, it is a reasonable detection model that screening positive samples by TP-ELISA and the positive samples are simultaneously detected by SYP. If the results of these two qualitative methods do not match, and then confirmed by TPPA. Finally, the non-specific antibody titer of the positive samples are detected by TRUST.
