摘要
目的探讨社区卫生服务利用程度和社区听力障碍老年人身心健康的关系。方法采用随机抽样的方法进行问卷调查,选取唐山市某社区430例听力障碍老年人作为研究对象,应用自制健康问卷测评听力障碍老年人对健康知识的知晓情况以及对健康知识、行为的影响;使用SRHMS自评量表对听力障碍老年人的身心健康进行测评。结合一般临床资料分析社区卫生服务利用情况和社区听力障碍老年人身心健康的关系。社区卫生利用以健康水平高低为因变量,以同时具有利用程度的高低水平组人群的年龄、性别、文化程度、居住情况、收入、婚姻质量、伴侣状况为自变量进行多因素Logistic回归分析。结果社区卫生服务利用程度低水平组听力障碍老年人群健康维度评分明显低于利用程度高水平组,差异具有统计学意义（P〈0.05）;社区卫生服务利用程度低水平组听力障碍老年人群的健康知晓率处于较差水平,对健康知识的知晓率为6.4%-28%。社区卫生服务利用程度高水平组听力障碍老年人群对健康知识的知晓率为38%-61.5%;社区卫生服务利用程度高水平组听力障碍老年人对健康方面的利用程度较高,知晓率〉50%,社区卫生服务利用程度低水平组听力障碍老年人对健康方面的知晓率较低,为20.1%-28.9%,两组差异具有统计学意义（P〈0.05）;文化程度高、居住环境良好、收入高及社区服务利用程度高是身心健康的保护性因素。高龄、婚姻质量差及丧偶者是危险性因素。结论社区卫生服务利用程度和听力障碍老年人身心健康息息相关,较高程度的社区卫生服务利用对听力障碍老年人身心健康有保护作用。
Objective To analysis the relationship between the utilization of community health service and health status for resident with hearing disorders of the elderly.Methods With layer-cluster random sampling,430 people with schizophrenia were assessed with health questionnaires,their demand for health service,to analysis the relationship between the utilization of community health service and health status for resident with hearing disorders of the elderly.Results The score in the health dimension on lower group was lower than the high group（P 〈0.05）;The high level group of related evaluation of health in community was high correct rate,mostly in the 38%-61.5% and the low level group is 6.4%-28%,there were significant difference between the two groups（P 〈0.05）;The results showed that the elderly population health awareness in the high level of health knowledge level were mostly in 50%in the community;The elderly population health awareness in the low level of health knowledge level were mostly respectively is 20.1%-28.9%,there were significant difference between the two groups（P 〈0.05）;Logistic regression showed that high education level,well living conditions,high income and community health service in the high degree were positive factor;an old age,marital quality and lost his or her partner were the risk factor（P 〈0.05）.Conclusion There was the relationship between the utilization of community health service and health status for resident with hearing disorders of the elderly.It is suggested that community health service in the high degree were positive factor.
