Cross-sectional study on influence of noise exposure on blood pressure of workers

摘要 目的了解噪声对作业工人血压的影响,为更好地加强管理,保护作业工人身心健康和为积极预防各类疾病的发生提供参考依据。方法将南通市2014年参加在岗期间职业健康检查的9 899名作业工人分为噪声接触与非噪声接触岗位,以听力损伤作为累计噪声暴露量的替代指标,对不同年龄和性别的工人高血压的发病情况进行对比研究。结果接触噪声的作业工人的高血压患病率(18.57%)与非接触噪声的作业工人的高血压患病率(19.37%)差异无统计学意义(P〉0.05)。以听力损伤为指标进行人口特征学分类后,有噪声所致的听力损伤者的高血压患病率明显高于听力正常的工人,其差异有统计学意义(P〈0.001),但仅限于男性。对接噪的男性作业工人进行年龄分层后,进一步进行logistic回归分析发现,有噪声所致的听力损伤者的高血压患病风险增高仅出现在〈35岁的年龄组(P=0.041,OR=1.113,95%CI:1.005-1.233),而在≥35岁的年龄组未发现这种相关性(P=0.071,OR=1.055,95%CI:0.995-1.119)。女性作业工人中未发现相关性。结论年轻作业工人中噪声致听力损伤与高血压有关,需重点关注。 [Objective]To understand the influence of noise exposure on blood pressure of workers, provide reference for strengthening the management, protecting the health of workers, and actively preventing the occurrence of diseases. [Methods]9 899 workers who have received the on-service occupational health examination in Nantong City in 2014 were divided into the noise exposure group and non-noise exposure group. The hearing loss was regarded as the substitutive indicator of cumulative noise exposure, to compare the prevalence of hypertension among different age, sex groups. [Results]The prevalence rate of hypertension in the noise exposure group and non-noise exposure group was 18.57% and 19.37% respectively, and the difference was not statistically significant( P〉0.05). The workers were classified for the demographic characteristics according to the indicator of hearing loss, the prevalence rate of hypertension in workers with noise-induced hearing loss was higher than that in workers with normal hearing, and the difference was statistically significant( P〈0.001), but this result was observed in males only. The male noise-exposed workers were stratified according to the age, the logistic regression analysis indicated that the risk of hypertension increased in 35 year-old age group of workers with noise-induced hearing loss( P =0.041, OR =1.113, 95% CI:1.005-1.233), while the correlation was not observed in ≥35 year-old age group( P=0.071, OR=1.055, 95% CI:0.995-1.119).There was no correlation in female noise-exposed workers. [Conclusion]The noise-induced hearing loss is related to the hypertension among younger noise-exposed workers, which should be focus on.
