摘要
目的探讨阻塞性睡眠呼吸暂停综合征（obstructive sleep apnea syndrome，OSAS）与原发性开角型青光眼（primary open angle glaucoma，POAG）的相关性。方法计算机检索PubMed、EMbase、Web of Science、WanFang Data、CNKI电子数据库，检索时间为建库到2014年11月，搜索研究OSAS与青光眼相关性的文献，由2名研究员按照纳入标准独立筛选文献、提取资料、质量评价并交叉核对，釆用Cochrane协作网提供的RevMan5.3软件对数据进行系统分析。结果共纳入4个病例对照研究和2个队列研究，共19 226例。OSAS与青光眼具有相关性（OR=2.03,95％CI为1.68-2.45,P〈0.01）。在不同国家亚组中,英国人OSAS与青光眼相关性不能确定（OR=0.87,95％CI为0.09-8.76,P=0.91）；土耳其人（OR=8.42,95％CI为1.05-67.41,P=0.04）和中国台湾人（OR=1.99,95％CI为1.64-2.40,P〈0.01）OSAS与青光眼有显著相关性。在不同程度OSAS亚组中，轻度OSAS患者（OR=3.13,95％CI为0.32-30.89,P=0.33）和中度OSAS患者（OR=5.16,95％CI为0.56-47.21,P=0.15）与青光眼相关性不能确定，重度OSAS患者（OR=11.30,95％CI为1.40-91.37,P=0.02）与青光眼有显著相关性。结论重度OSAS可能为POAG发病的危险因素。
Objective To systemically review the correlation of obstructive sleep apnea syndrome （OSAS）and primary open angle glaucoma （POAG）.Methods Literatures concerning the correlation between OSAS and glaucoma were searched in PubMed, EMbase, Web of Science, WanFang Data and CNKI （from the date of the databases founded to November 2014）. Two researchers did systematic analysis to the data with RevMan5.3 software provided by the Cochrane Collaboration throughout screening the literatures independently according to the inclusive criteria, extracting the data, assessing the quality and cross checking. Results Four case-control studies and 2 cohort studies were included in this article, totally 19 226 participants. The results showed that OSAS was related with glaucoma （OR=2.03, 95%CI=1.68-2.45,P〈0.01）. In subgroups according to the race and nation analysis, the correlation between OSAS and glaucoma was determined for the Turk （OR=8.42, 95%CI=1.05-67.41, P=0.04） and Taiwannese （OR=1.99, 95%CI=1.64-2.40, P〈0.01）, but not for the British （OR=0.87, 95%CI=0.09-8.76, P=0.91）. In the subgroups according to the severity of OSAS, the correlation was certain in the patients with severe OSAS （OR=11.30, 95%CI=1.40-91.37, P=0.02）, but not in those with mild （OR=3.13, 95%CI=0.32-30.89, P=0.33） and with moderate OSAS （OR=5.16, 95%CI=0.56-47.21, P=0.15）. Conclusion OSAS might be a risk factor for POAG.
