摘要
目的探讨肾虚老年性痴呆中医证候的临床表现特点。方法对570例肾虚老年性痴呆患者中医证候资料进行因子分析、聚类分析。结果因子分析发现肾虚证老年性痴呆以精亏、阳虚、阴虚为主要证候要素,同时兼夹瘀、痰、火、血虚、气虚等致病要素,临床表现为肾虚证兼夹瘀血、痰浊、心肝火旺等。结合临床专业知识聚类分析发现三类证候分布:第1类以肾精亏虚为主,未发现其他兼夹证候;第2类以肾阴虚为主兼夹心肝火旺证及心脾两虚证;第3类以肾阳虚为主兼夹气滞血瘀证及心脾两虚证,并且肾精亏虚证所占比例最大。结论肾虚老年性痴呆以肾精亏虚、肾阳虚、肾阴虚为主要证候,兼夹瘀、痰、火等致病因素,五脏与心、肝、脾密切相关,病机与精亏密切相关。
Objective To explore the clinical performance of kidney empty senile dementia syndromes. Methods 570 cases of kidney patients with senile dementia syndromes data factor analysis and cluster analysis. Results Factor analysis found that kidney deficiency of senile dementia in Jing Kui,Yang deficiency,Yin deficiency as the main syndrome factors,at the same time and blood stasis,phlegm,fire and blood deficiency,Qi deficiency such as pathogenic factors,clinical manifestations of kidney deficiency and blood stasis,phlegm turbidity,heart the fire,and so on. Combined with clinical knowledge cluster analysis found that three types of syndrome distribution： the first kind is given priority to with kidney essence deficiency,found no other and clamp syndrome; The second kind is given priority to with kidney Yin deficiency and sandwich something syndrome and glove two deficiency syndrome; The third kind is given priority to with kidney Yang deficiency and clip two deficiency syndrome,qi and blood stasis and glove and the largest proportion of kidney essence deficiency syndrome. Conclusion Kidney empty senile dementia in kidney essence deficiency,kidney Yang deficiency and kidney Yin deficiency as the main syndrome,and clip stasis,phlegm and pathogenic factors,such as fire five Zang- organs is closely related to the heart,liver,spleen,pathogenesis is closely related to Jing Kui.
