摘要
目的观察电针联合揿针对肥胖并发高脂血症胃肠腑热型患者的良性作用。方法选择肥胖并发高脂血症胃肠腑热型患者112例,随机分为治疗组和对照组,各56例,治疗组采用电针联合揿针施治,对照组采用单纯电针施治,并与正常人52名(正常组)作对比,观察正常组及治疗组和对照组治疗3个月前后患者肥胖指标、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素敏感性指数(ISI)、胰岛素抵抗指数(Homa-IR)、胰岛β细胞功能指数(Homa-β)、空腹瘦素水平(FLP)及植物神经功能指数(Y值)的变化和疗效。结果治疗组与对照组患者治疗前肥胖指标、TC、TG、LDL-C、FPG、FINS、Homa-IR、FLP均显著高于正常组(P<0.01),而HDL-C、ISI、Y值显著低于正常组(P<0.01)。针灸治疗后两组患者均取得了较好的减肥疗效,脂质指标:TC、TG、LDL-C水平均明显回降,HDL-C水平却明显回升;"脂肪-胰岛内分泌轴"指标:FPG、FINS、Homa-IR、FLP均明显回降,而ISI和Y值均明显回升(P<0.01)。治疗组在血脂水平、脂肪-胰岛内分泌轴以及植物神经功能的调整作用优于对照组(P<0.05)。结论胃肠腑热型肥胖并发高脂血症患者存在脂质代谢、植物神经和"脂肪-胰岛内分泌轴"功能紊乱;电针联合揿针和单纯电针治疗均取得了很好的减肥效应,同时它对脂质水平、脂肪-胰岛内分泌轴及植物神经的功能具有良性调整作用,其作用电针联合揿针法优于电针法。
Objective To observe the positive effect of electro- acupuncture combined with embedding needle in obesity complicated with hyperlipidaemia patients with stomach- intestine excessive heat type. Methods Choose 112 patients with obesity complicated with hyperlipidaemia patients with stomach- intestine excessive heat type,and randomly divided into treatment group of56 cases and blank group of 56 cases. Treatment group were given combo therapy of electro- acupuncture and embedding needle,blank group were given electro- acupuncture only,and compare with 52 normal people（ normal group）. The changes in obesity index,total cholesterol（ TC）,trigly ceride（ TG）,low density lipoprotein（ LDL- C）,high density lipoprotein（ HDL- C）,the fasting plasma glucose（ FPG）,fasting insulin（ FINS） fasting leptin（ FLP）,insulin sensitivity index（ ISI）,insulin resistance index（ Homa- IR）,β- cell function index（ Homa- β） and vegetative nerve system equilibrium index（ Y value） in patients were observed before the treatment and 3 months after treatment. Results The obesity index,TC,TG,LDL- C,FPG,FINS,Homa- IR and FLP of treatment group and blank group were significantly higher than normal group（ P〈0. 01）,while HDL- C,ISI,Y value of those two groups were significantly lower than normal group（ P〈0. 01）. After treatment,treatment group and blank group both obtained promising effect in reducing weight,at the same time,the obesity index and levels of TC,TG,LDL- C,FPG,FINS,Homa- IR and FLP significantly decreased,while HDL- C,ISI and Y value significantly increased（ P〈0. 01）. The regulatory effect on lipid level,fatty- insulin endocrine axis and vegetative nerve function of treatment group was better than blank group（ P〈0. 05）. Conclusion There is dysfunction of lipid metabolism,fatty- insulin endocrine axis and vegetative nerve in obesity complicated with hyperlipidaemia patients with stomach- intestine excessive heat type.
