摘要
目的观察人类免疫缺陷病毒(HIV)视网膜病变的眼底改变和荧光素眼底血管造影(FFA)特征。方法于2011年9月至2014年9月在郑州市第二医院在眼科就诊并经临床和实验室检查确诊为HIV视网膜病变的患者25例38只眼纳入研究。所有患者均进行常规视力、裂隙灯显微镜检查、直接眼底镜检查及FFA检查,对其眼底病变的临床特征进行回顾性分析。结果25例38只眼中,视力〈0.3者2只眼,0.3-〈0.8者8只眼,≥0.8者28只眼。视网膜棉絮斑24只眼,占63%;表现为后极部血管旁或视盘旁单个或多个棉絮斑。视网膜出血8只眼,占21%;表现为后极部出血斑片或Roth斑。视网膜微血管瘤6只眼,占15%。5例8只眼随访一年,其中2例4只眼原有出血或棉絮斑消失,1例2只眼出现新的棉絮斑或出血,2例2只眼原有棉絮斑病变发展为巨细胞病毒性视网膜炎。结论棉絮斑是HIV视网膜病变最常见的眼底表现;FFA对HIV视网膜病变有一定诊断价值。
Objective To investigate the fundus characteristics and fundus fluorescence angiography (FFA) features of human immunodeficiency virus (HIV) retinopathy. Methods Twenty-five HIV patients (38 eyes) received examinations of visual acuity, slit-lamp microscope, ophthalmoscope and FFA. The results were retrospectively analyzed. Results Visual acuity was as following: 〈0.3, 2 eyes; 0.3-0.8, 8eyes; ≥0.8, 28 eyes. There were three types of ocular findings, including cotton-wool spots (24 eyes, 63%), hemorrhage (8 eyes, 21%) and microaneurysm (6 eyes, 15%). Among 8 eyes of 5 followed-up patients, cotton-wool spots or hemorrhage disappeared without treatment in 4 eyes of 2 patients, and new cotton-wool spots or hemorrhage were found in 2 eyes of 1 patient. 2 eyes of 2 patients considered as cotton-wool spots were detected with cytomegalovirus retinitis. Conclusions Cotton-wool spots are the most common ocular manifestations in HIV retinopathy.
