摘要
目的观察大剂量生地凉血汤即犀角地黄汤中加用大剂量生地治疗脓疱型血热证银屑病的临床疗效及对外周血肿瘤坏死因子-α(TNF-α)水平的影响。方法观察予大剂量生地凉血汤口服治疗的30例血热证银屑病患者治疗前后皮损面积和严重性指数(Psoriasis area and severity index,PASI)评分,中医症状评分的变化,并采用酶联免疫吸附试验法(ELISA)检测治疗前、治疗4周后血清TNF-α的水平。结果大剂量生地凉血汤能明显改善脓疱型血热证银屑病的临床症状,治疗后PASI评分,各中医症状评分较治疗前均有明显改善(P<0.05),治疗4周后血清TNF-α水平下降明显,与对照组相比,差异有统计学意义(P<0.05),且TNF-α与PASI评分呈正相关(r=0.39,P<0.05)。结论大剂量生地凉血汤可通过降低血清TNF-α水平,从而起到治疗银屑病的作用,且血清TNF-α与病情密切相关,可作为评价银屑病活动程度的参考指标。
Objective To observe the clinical efficacy on pustular psoriasis differentiated as blood heat pattern treated with blood-cooling soup with large doses of Shengdi( Xijiao Dihuang Tang uesed with large doses of Shengdi) and impacts on TNF-α.Methods 30 cases of pustular psoriasis differentiated as blood heat pattern were treated with oral administration of blood-cooling soup with large doses of Shengdi. The changes in the score of psoriasis area and severity index(PASI) and the scores of symptoms of traditional Chinese medicine(TCM) were observed before and after treatment. ELISA was adopted to detect the levels of serum TNF-α before treatment and in four weeks after treatment separately. Results Blood-cooling soup with large doses of Shengdi relieved apparently the clinical symptoms of pustular psoriasis and improves PASI score and the scores of the symptoms in TCM after treatment as compared with those before treatment(P〈0.05). The levels of TNF-α were reduced apparently in four weeks of treatment,indicating the significant difference as compared with control group(P〈0.05). And the positive correlation presented as compared with PASI score respectively(r=0.39,P〈0.05). Conclusion Blood-cooling soup with large doses of Shengdi can reduce the levels of serum TNF-α, which plays the role in treating psoriasis. This index is closely related to the condition of disease and can be taken as the reference for the assessment of the activity of psoriasis.
