摘要
目的根据椎动脉彩色多普勒及频谱多普勒表现,结合锁骨下动脉或无名动脉有无狭窄及患侧上肢动脉血流变化,探讨彩超诊断锁骨下动脉窃血综合征的临床意义。方法 29例锁骨下动脉窃血综合征患者分别进行锁骨下动脉、颈动脉、椎动脉、上肢动脉血流方向、速度、频谱特征等比较,根据椎动脉血流频谱表现将其分为三型:隐匿型(收缩期出现切迹)、部分型(收缩期血流反向,舒张期血流正向)、完全型(全心动周期血流均反向)。结果病变左侧多见,病因以动脉硬化为主,约86.2%(25/29),大动脉炎3例占10.3%,动脉发育畸形1例占3.4%;完全型窃血9例,部分型15例、隐匿型5例。患侧上肢动脉血流流速正常或减低、频谱呈单峰或低慢波。结论彩超能对绝大多数锁骨下动脉窃血综合征能明确诊断,窃血程度与锁骨下动脉或无名动脉狭窄程度有关。
Objective According to the vertebral artery color doppler and spectral doppler performance, combined with the subclavian artery or unknown that whether had narrow and risk of upper extremities artery blood flow changes, to dis cusses color dopplor ultrasound in the diagnosis of subclavian artery steal syndrome. Methods We comparatively analyzed the 29 cases of subsclavian steal syndrome, which included direction and velocity of blood flow and spectrum signature in subclavian artery, vertebral artery and artery of upper extremity. Arterial steal was divided into three types： occult type （Systolic incisure appeared ）, partial type （reverse flow in systole and forward flow in diastole） and complete type （reverse flow in the cardiac cycle） according to the spectrum characteristics of blood flow of vertebral artery. Results The lesions were mostly found in left side and arteriosclerosis mainly caused the disease, about 86.2% （25/29）, including takayasu arteritis in 3 cases, artery malformation in 1 case; complete steal in 9 cases, partial steal in 15 cases and occult steal in 5 cases. The Arterial blood flow of upper extremity in the affected side mainly registered as the normal speed or reduction; spectra occured unimodal pattern or low slow wave. Conelusion Color Doppler ultrasound can positively diagnose the majority of SSS, the degree of steal is associated with the level of angiostenosis.
