摘要 目的 :探讨经尿道双极电切的安全性与有效性。方法 :该法使用生理盐水为冲洗液、无负极板。用经尿道双极电切行前列腺电切 (双极TURP) 15 0例 ,病人年龄 6 1~ 85岁 ,平均 71.5岁 ;前列腺体积 2 1~ 112 (49.5± 32 .7)g,术前带尿管者 39例 ,术后随访 1~ 3月。结果 :双极TURP手术时间 16~ 93(39± 2 9)min ,切除前列腺组织重量平均 7~ 90 (31± 2 6 )g ,无 1例需输血 ,无 1例TUR综合征发生 ;术后IPSS、QOL、PFR显著改善。结论 :双极电切进行TURP是一种新的安全。 Objective: To assess the efficiency and safety of transurethral bipolar electrocautery resection of the prostate(Bipolar TURP). Methods: The bipolar electrocautery system permits rapid tissue removal with little bleeding and no pad return using saline irrigation. 150 patients(mean age 71.5; range 61~85) with symptomatic BPH without suspected cancer(confirmed by IPSS, urinary flow rate, ultrasonographic estimate and PSA tests) were treated by bipolar TURP. All patients were followed up in 1~3 months postoperatively. Results: The duration of the procedure was (39±23)min(range 16~89 min). No case needed blood transfusion during the operation. No TUR syndrom occurred. The mean time of postoperative continuous irrigation was 1day(0.5~2 d). The mean catheterization time was 2 days(1~4 days). The peak flow rate increased from (5.1±3.8 )to (19.1±3.2), (21.7±4.2)ml/s at 1 and 3 months respectively. The IPSS decreased from 24.6 to 5.7, 5.3 at 1 and 3 months respectively. Conclusion: Transurethral bipolar electrocautery resection of the prostate is effective and safe. This pilot series permits a comparative study with conventional TUR system.
