摘要
目的探讨活血通腑方(大黄、桃仁、芒硝、延胡索、莱菔子、红花)对CD40、CD40L、单核细胞趋化蛋白-1(MCP-1)的影响及其对大鼠术后腹腔粘连的影响。方法选用SD雄性大鼠150只,采用锉刀法建立术后腹腔粘连大鼠,随机均分为模型组、活血通腑方低、中和高剂量组、四磨汤组5组,另取30只为假手术组。灌胃治疗2周,分别于实验第1、3、5、7和14天,观察腹腔粘连评分和粘连组织病理变化来评价疗效,同时采用流式细胞术检测其外周血CD40、CD40L表达,免疫组化检测粘连组织CD40、CD40L、MCP-1蛋白水平。结果与模型组比较,活血通腑方中、高剂量组肉眼腹腔粘连评分和镜下评分在术后第3天和第7天明显低于模型组,均有显著性差异(P<0.01)。各组在不同时间点能不同程度降低外周血中CD40和CD40L的水平;同时降低粘连组织中CD40、CD40L和MCP-1蛋白表达水平(P<0.05)。结论活血通腑方改善腹腔粘连程度的药理机制可能是有效抑制CD40/CD40L活化作用,降低MCP-1的表达,减少机体免疫及验证反应。
AIM To explore the effects of Huoxue Tongfu Formula( Rhei Radix et Rhizoma,Persicae Semen,Natrii sulfas,Corydalis Rhizoma,Raphani Semen,Carthami Flos)( HXTFF) on MCP-1,CD40 and CD40 L in rats with postoperative peritoneal adhesion( PPA) and their mechanisms of action. METHODS One hundred and fifty male rats were used to establish postoperative peritoneal adhesion model by filing operation and divided randomly and equally into five groups: the model group,low,middle,high dose HXTFF groups and Simo Decoction group; another thirty SD rats served as sham operation group. Each group was treated according to the prescribed dose by gavage for two weeks. The animals were euthanized on the first,3rd,5th,7th and the 14 th day to evaluate the adhesion severity,area of adhesion formation,gross appearance,and pathological changes. Furthermore,immunohistochemical investigations were conducted to determine the influence of HXTFF on peripheral blood and peritoneal tissue responses( MCP-1,CD40 and CD40L). RESULTS Adhesion scores and pathological changes from HXTFF groups were down-regulated as compared with those of the model group,especially on the 3rd and the 7th day after operation( P〈0. 01). In comparison with the model group,the levels of CD40 and CD40 L in peripheral blood decreased to a certain extent in all treated groups( P〈0. 05). While the levels of CD40,CD40 L and MCP-1 protein expression in tissue adhesion in the four treated groups were lower than those in the model group( P〈0. 05). CONCLUSIONS The pharmacological mechanism of Huoxue Tongfu Formula improving abdominal adhesion may be through effectively inhibiting CD40 / CD40 L activation and decreasing MCP-1's expression.
