骨显像眼眶单发放射性浓聚灶分析

摘要 目的分析骨显像眼眶单发放射性浓聚灶的特点及临床意义。方法回顾性分析30例经全身骨显像检出眼眶单发放射性浓聚灶患者的骨显像及相关影像学检查资料,观察眼眶放射性浓聚灶的位置、形态,通过随访结合CT、MR检查对照分析病灶特点。结果 30例中,骨显像示眼眶放射性浓聚灶位于右侧17例,左侧13例;累及眼眶上壁23例,外壁4例,下壁2例,内壁1例;呈点状放射性浓聚28例,呈条状放射性浓聚2例。10例接受骨显像随访复查的患者均未见新发病变,其中8例眼眶病灶较前无明显变化,2例较前变浅。10例接受颅脑CT检查的患者中,8例呈磨玻璃密度,2例呈低密度;7例密度均匀,3例密度不均;7例病灶最大径<1cm,3例≥1cm。5例病灶呈膨胀性改变。9例接受颅脑MR检查的患者中,6例MR未见异常,3例T1WI及T2WI可见眼眶局灶性低信号。结论眼眶单发浓聚灶好发于外上壁,多呈点状放射性浓聚,CT及MR检查提示部分为骨纤维源性病变。 Objective To investigate the manifestations and clinical value of solitary orbital lesions on 99 Tcm-MDP imaging.Methods The imaging data of 30 patients with solitary hot lesion on bone scan were retrospectively analyzed.The location and shape of lesions were observed,and the features were analyzed by combination of bone scan follow-up,CT and MR examines.Results There were 17 cases with lesions located on right orbit and 13 cases with lesions located on the left.Lesions located on superior,lateral,inferior and medial orbit were found in 23,4,2and 1case,respectively.99 TcmMDP bone scan showed punctuate increased uptake in 28 cases and striped uptake in 2cases.No new lesions were found in all of 10 patients underwent bone scan follow-up.Among them,orbital lesions were showed decreased uptake than before in 2cases and stable in 8cases.In 10 patients underwent brain CT,ground-glass density were found in 8cases and hypodensity were found in another 2cases,as well as homogeneous density were found in 7cases and heterogeneous density were found in 3cases.There were 7cases with the diameter of lesion 〈1cm and 3cases with the diameter of lesion ≥1cm.The expansile lesions were found in 5cases.In 9patients underwent brain MRI,there were 6cases without detectable lesion and 3cases of hypointensity lesions on T1 WI and T2 WI.Conclusion Most solitary orbital lesions detected on bone scan are punctate and locate on superior or lateral wall.Some of them may be fibro-osseous lesions indicated by CT and MRI.
