摘要
目的:探讨多层螺旋CT尿路成像(CTU)在输尿管疾病中的临床应用价值.材料与方法:搜集本院2013年7月~2015年2月短期内(1周)进行B超、CTU检查并明确诊断的输尿管疾病患者36例.所有患者MSCTU检查均行四期扫描,包括:平扫、皮质期、髓质期和排泄期,并将扫描获得的容积数据传送至后处理工作站进行图像重建.①以手术和病理诊断或临床治疗结果为金标准,比较B超与CTU两种检查方法对输尿管疾病诊断的定位诊断符合率与定性诊断符合率.②以手术和病理诊断或临床治疗结果为对照,分析MSCTU四期扫描获得的单纯轴位图像与轴位图像结合重建图像的影像表现,比较两种观察手段在诊断输尿管病变中的定位诊断符合率及定性诊断符合率情况,进一步评估CTU图像重建技术在输尿管疾病中的诊断价值.结果:①B超和CTU对输尿管疾病中的定位诊断符合率为分别为67%、97%,两者的定位诊断符合率差异有统计学意义(X2 =9.39,p<0.05);两者的定性诊断符合率分别为58%和89%,差异有统计学意(X2 =7.15,p<0.05).②CTU中单纯的轴位图像在输尿管疾病中的定位及定位诊断符合率为分别为78%、72%,低于轴位+三维重建图像(97%、94%),差异均有统计学意义(X2 =4.57和6.40,P<0.05).结论:CTU对于输尿管疾病的诊断能力明显优于B超,对输尿管疾病的诊断具有重要的价值.CTU及图像后处理技术能对输尿管病变进行多方位、多角度观察,为轴位图像提供了许多补充信息,轴位图像与重建图像综合应用可以提高输尿管疾病的诊断准确率.
Purpose: To evaluate the application of multi-slice spiral CT urography (CTU) in the diagnosis of ureter- al diseases. Materials and Methods: From July 2013 to February 2015, 36 patients with ureteral diseases were examined by Ultrasound (US)and CTU in our hospital. All patients were performed by CTU examination with four phase scannings( inclu- ding plain scan, cortical phase, parenehymal phase and Excretory phase). The volume data were transferre to the post-pro- cessing workstation for image reconstruction. (~)Comparing the localization diagnosis and qualitative diagnosis coincidence rate of two methods in diagnosis of ureteral diseases, to assess the value ofCTU in the diagnosis of ureteral diseases. (~)Analys- izing the pure axial image of CTU four phase scanning and the image of axial images combined with reconstruction image, com- pared the localization diagnosis and qualitative diagnosis coincidence rate of two kinds of observation methods, and compared with operation and pathology or clinical treatment results, to further assess the value of CTU image reconstruction technique in the diagnosis of ureteral diseases. Results: ①The location diagnosis coincidence rate of US and CTU in ureteral disease were 67% and 97%, the differences was statistically significant (X2 = 9. 39, p 〈 0.05 ) ; The qualitative diagnosis rate were 58% and 89%, respectively, the differences was statistically significant (X2 = 7. 15, p 〈 0. 05 ) . The location and qualitative di- agnosis rate of CTU was significantly higher than US .
