摘要
目的探讨中西医结合对不同证型帕金森并抑郁的疗效及认知功能的影响。方法选择2014年1月-2015年1月我院确诊的60例帕金森合并抑郁患者作为观察对象，全部患者随机分为观察组和对照组各30例．对照组单用文拉法辛，观察组同时联合中药汤剂进行治疗，比较两组治疗3个月后的抑郁程度、不同证型的疗效及认知功能MMSE评分。结果治疗后，观察组总有效率为90．0％，对照组总有效率为60．0％，观察组总有效率显著高于对照组（P〈0．05）。观察组不同证型的疗效比较，差异无统计学意义；对照组肝肾不足型的有效率显著高于其他分型（P〈0．05）。观察组痰浊阻滞型、瘀血阻络型的有效率分别显著高于对照组（P〈0．05）。两组痰浊阻滞型、瘀血阻络型、肝肾不足型不同证型患者治疗后的MMSE评分均较治疗前显著降低，且观察组较对照组降低更显著（P〈0．05）。但同组无论治疗前后不同证型的认知功能MMSE评分组间比较，差异无统计学意义（P〉0．05）。结论中西医结合对帕金森并抑郁的疗效显著优于单独西医治疗，且可以减轻患者的抑郁程度，改善患者的认知功能，但不同证型间差异不大。
Objective To investigate the curative effect of traditional Chinese medicine and western medicine in treat- ment of Parkinson with depression. Methods From January 2014 to January 2015, 60 cases of Parkinson＇s disease with depression were randomly divided into observation group and control group, 30 cases in each group, two groups of pa- tients received conventional drug treatment, control group was treated with single venlafaxine, observation group was combined with decoction of Chinese medicine treatment, depression, different syndromes efficacy and cognitive function MMSE score were compared between the two groups after 3 months treatment. Results After treatment, the total effi- ciency of observation group was 90.0%, the control group was 60.0%, the total efficiency of the observation group was significantly higher than that of the control group, the difference was statistical significant （P〈0.05）. The difference of different syndromes was not significant, and the efficiency of the liver and kidney deficiency in the control group was significantly higher than that of the other types （P〈0.05）. The efficacy of phlegm type, blood stasis type in observation group were significantly higher than those in control group（P〈0.05）. The MMSE score of turbid phlegm type, blood stasis type, liver and kidney deficiency type of two groups after treatment were significantly reduced than before treatment, and the observation group decreased significantly than the control group （P〈O.05）. However, the difference of the cogni- tive function of MMSE between the two groups before and after treatment was not significantly（P〉0.05）. Conclusion Traditional Chinese combined with western medicine in threatment of Parkinson＇s disease and depression＇s curative ef- fect is better than that of western medicine alone, and can reduce the degree of depression, improve cognitive function in patients, but there is no significant difference between different TCM syndrome types.
