摘要
目的观察并比较常规治疗及其联合曲美他嗪治疗糖尿病合并冠心病慢性充血性心力衰竭的疗效。方法以近年来该院收治的60例糖尿病合并冠心病慢性充血性心力衰竭患者为研究对象,按照随机化将患者分为对照组和观察组,前者给予常规治疗,后者加用曲美他嗪,观察并比较两组患者治疗1年内的再住院率和病死率,观察并比较两组患者治疗4周和1年的左心室功能(评价指标包括射血分数、收缩末期和舒张末期容积指数)、6 min步行距离和24 h心肌缺血发作次数。结果对照组1年内的再住院率为18.33%,病死率为13.33%;观察组再住院率为5.00%,病死率为3.33%,两组患者再住院率和病死率之间的差异有统计学意义(P<0.05)。两组患者治疗后,提高了左心室的射血分数,降低了左室收缩和舒张末期容积,曲美他嗪组与常规治疗组比较差异有统计学意义(P<0.05)。结论曲美他嗪治疗糖尿病合并冠心痛慢性CHF患者1年后的心功能和心肌缺血改善优于4周时的改善情况,服药1年明显改善了患者的心脏功能,提高了生活质量,显著降低了患者的再住院率和病死率,疗效安全满意,值得临床广泛推广应用。
Objective To observe and compare conventional treatment and combined with trimetazidine trimetazidine treat-ment of diabetes mellitus complicated with coronary heart disease and chronic congestive heart failure curative effect. Methods In recent years was admitted to our hospital with 60 cases of diabetes mellitus complicated with coronary heart disease and chronic congestive heart failure patients as the research object, according to the randomized the patients were divided into the observation group and the control group, the former giving routine treatment, the latter with trimetazidine trimetazidine, observe and compared two groups of patients with treatment within one year rehospitalization rate and mortali-ty rate, observed and compared two groups of patients with treatment for 4 weeks and 1 year of left ventricular function （in-cluding evaluation index shot ejection fraction and end systolic and end diastolic volume index）, 6-minute walking distance and 24h myocardial ischemia episodes. Results In the control group, the hospitalization rate was 18.33%, the mortality rate was 13.33%, the mortality rate was 5%, the mortality rate was 3.33%, the difference between the two groups was statistical-ly significant（P〈0.05）. After treatment, the patients in the two groups, improve the left ventricular ejection fraction, left ven-tricular systolic and end diastolic volume is reduced, the trimetazidine group and routine treatment group difference has sta-tistical significance（P〈0.05）. Conclusion Trimetazidine trimetazidine in the treatment of diabetes mellitus complicated with coronary heart patients with chronic congestive heart failure （CHF） after a year of cardiac function and myocardial ischemia is better than 4 weeks when the improvement of medication 1 year can significantly improve the heart function in patients with, improve the quality of life, which can significantly reduce the patients rehospitalization rate and mortality rate, safe and effective satisfaction, it is worth to be widely ap
