低T3综合征辨证分型与血清TT3、FT3水平关系的研究 被引量:10

摘要 目的:研究低T3综合征辨证分型与血清TT3、FT3的相关性,为低T3综合征的中医辨证提供客观依据。方法:选择50例健康体检者为对照组,收集169例低T3综合征患者为低T3综合征组,对其临床症状、体征、甲状腺激素水平,借助现代统计学方法研究低T3综合征辨证分型与血清TT3、FT3的相关性。结果:低T3综合征组中气血两虚证16例、脾肾阳虚证87例、心肾阳虚证54例,肝肾阴虚证12例,其中脾肾阳虚证87例占51.48%,比例最高;心肾阳虚证组TT3、FT3水平最低,与气血两虚证组、脾肾阳虚证组、肝肾阴虚证组相比差异均有统计学意义(P<0.05);心肾阳虚证组的阳虚症状积分明显高于气血两虚证组、脾肾阳虚证组、肝肾阴虚证组,差异有统计学意义(P<0.05)。结论:低T3综合征中阳虚证占首位,其中脾肾阳虚证所占比例最高,低T3综合征阳虚程度越重,TT3、FT3水平越低,TT3、FT3水平的变化有助于中医辨证。 Objective : To study the relationship between the TCM syndrome differentiation of low T3 syndrome and the serum TT3 and FT3 in order to provide the evidence of TCM syndrome differentiation of low T3 syndrome. Methods:50 healthy volunteers were selected as control group, observing general patient data, symptoms, the level of thyroid hormone to compare with 169 patients in low T3 syndrome group to research the relationship between the TCM syndrome differentiation of low T3 syndrome and the serum TT3 and FT3 by statistical method. Results : 169 cases of low T3 syndrome group were divided into 4 types: 16 cases of syndrome of deficiency of both qi and blood,87 cases of syndrome of yang deficiency of spleen and kidney,54 cases of syndrome of yang deficiency of heart and kidney and 12 cases of syndrome of yin deficiency of liver and kidney. Syndrome of yang deficiency of spleen and kidney(51.48% ) was the most common TCM syndrome in low T3 syndrome group. TT3 and FT3 in syndrome of yang deficiency of heart and kidney were lower than those in syndrome of deficiency of both qi and blood, syndrome of Yang deficiency of spleen and kidney and syndrome of yin deficiency of hver and kidney( P 〈0.05 ). The TCM pattern score of yang deficiency in syndrome of yang deficiency of heart and kidney group was higher than those in syndrome of deficiency of both qi and blood, syndrome of yang deficiency of spleen and kidney and syndrome of yin deficiency of liver and kidney( P 〈 O. 05 ). Conclusion:Yang deficiency syndrome was the most common TCM syndrome in low T3 syndrome group. The most common yang deficiency syndrome was syndrome of yang deficiency of spleen and kidney. More serious the yang deficiency of low T3 syndrome was,the lower of FT3 levels. The changes of thyroid hormone level may help to judge the TCM syndrome differentiation of low T3 syndrome.
