摘要
目的 探讨经鼻电子喉镜诊治疑难性咽喉异物的特点。方法 回顾性分析2014年1月至2015年1月经鼻电子喉镜诊治疑难性咽喉异物364例,根据年龄分为四组:未成年组(〈18岁),青年组(18~39岁),中年组(40~59岁),老年组(≥60岁)。结果 364例咽喉异物中,98.6%为鱼刺(359例),1.4%为其他异物;不同年龄组所占比例由高到低依次为:青年组为59.9%,中年组26.4%,老年组9.3%,未成年组4.4%。咽喉异物常见的停留位置主要为:舌根56.0%,会厌谷15.4%,扁桃体13.2%,梨状窝10.4%,咽后壁2.7%;364例咽喉异物中,359例电子喉镜下使用配套活检钳成功取出异物,3例异物需使用喉异物钳自口腔取出,2例扁桃体鱼刺经电子喉镜检查发现后在间接喉镜下取出。结论 疑难性咽喉异物的异物种类主要为鱼刺;患者多为青年人;异物常见停留位置主要为舌根,会厌谷,扁桃体,梨状窝;经鼻电子喉镜诊治疑难性咽喉异物准确有效。
Objective To explore the characteristics of diagnosis and treatment for difficult throat foreign bodies by trans-nasal electronic laryngoscope. Methods 364 cases of difficult throat foreign bodies from January 2014 to January 2015 were analyzed retrospectively. All cases were diagnosed and treated by trans-nasal electronic laryngoscope, and divided into four groups according to age: minor group (below 18 years old), young group (18 ~ 39 years old), middle-aged group (40 ~ 59 years old), elderly group (aged over 60). Results Among 364 cases of throat foreign bodies, there were 359 cases (98.6%) offishbone. Among different age groups, the rate of throat foreign bodies from high to low was followed: the young group (59.9%), the middle age group (26.4%), senile group of (9.3%), and minor group (4.4%). The major locations of throat foreign bodies were base of tongue (56.0%), epiglottic vallecula (15.4%), tonsil (13.2%), pyriform fossa (10.4%), posterior wall of pharyngeal (2.7%). 359 cases foreign bodies were removed successfully under electronic laryngoscope by biopsy forceps, 3 cases of foreign bodies were removed via oral by laryngeal foreign body forceps, and 2 cases of fishbone in tonsil were removed using indirect laryngoscope after detected by electronic laryngoscope. Conclusions The main type of foreign body in difficult throat foreign bodies is fishbone. The patients are mostly young people. It is accurate and effective for diagnosis and treatment of difficult throat foreign bodies by trans-nasal electronic laryngoscope.
