摘要
目的探讨128层螺旋CT冠状动脉成像技术在冠心病患者中的应用价值。方法以我院放射科收治的124例冠心病患者作为研究对象,根据其临床资料进行回顾性分析和总结。患者均进行螺旋CT冠状动脉成像检查和冠状动脉造影检查,以冠状动脉造影检查结果为金标准,分析128层螺旋CT冠状动脉成像诊断冠心病的敏感性、特异性等。结果冠状动脉造影显示冠状动脉轻度狭窄58例,中度狭窄62例,中度狭窄52例,闭塞17例;128层螺旋CT冠状动脉成像显示冠状动脉轻度狭窄67例,中度53例,重度46例,闭塞17例,两种诊断方式相比,差异无统计学意义(P>0.05);螺旋CT冠状动脉成像诊断冠状动脉狭窄的敏感性为95.9%,特异性为96.3%,阳性预测值为92.3%,阴性预测值为97.9%。结论 128层螺旋CT冠状动脉成像技术在冠心病的诊断中具有重要价值,可以和冠状动脉造影相媲美,具有较高的敏感性、特异性,能提高诊断率,是一种无创、完全的检查方式,为诊断冠心病的理想选择。
Objective This paper is to explore the application value of 128-slice spiral CT coronary artery imaging in patients with coronary heart disease (CHD).Methods One hundred and twenty four patients with coronary heart disease (CHD) admitted in the Radiology Department of the Hospital were selected as research subjects, and retrospective analysis and summary were made according to clinical data. All patients were subject to spiral coronary CT artery imaging and coronary artery angiography, coronary angiography result was treated as the gold standard, the sensitivity and specificity of 128-slice spiral CT coronary artery imaging in the diagnosis of coronary heart disease (CHD) were analyzed. Results Coronary angiography showed 58 patients had mild coronary artery stenosis, 62 patients had moderate stenosis, 52 patients had moderate coronary artery stenosis, and 17 patients suffered from occlusion. 128-slice spiral CT coronary artery imaging showed 67 patients had mild coronary artery stenosis, 53 patients suffered from moderate coronary artery stenosis, 46 patients had severe coronary artery stenosis, 17 patients suffered from occlusion, there was no statistically significant difference (P〉0.05) when two types of diagnostic methods were compared. The sensitivity and specificity of spiral CT coronary artery imaging in the diagnosis of coronary artery stenosis hit 95.9% and 96.3% respectively, positive predictive value hit 92.3%, and negative predictive value hit 97.9%.Conclusion 128-slice spiral CT coronary artery imaging delivers important value in the diagnosis of coronary heart disease, and it is comparable to coronary angiography. This technique is provided with high sensitivity and specificity, and it can improve the diagnostic rate, becoming a type of noninvasive and complete inspection way and ideal option for the diagnosis of coronary heart disease (CHD).
