摘要
目的分析高刺激率听性脑干反应(ABR)在梅尼埃病和前庭性偏头痛患者中的表现,为二者的鉴别诊断提供依据。方法回顾性分析2012年4月~2014年4月武警总医院眩晕病研究所收治的56例梅尼埃病患者与37例前庭性偏头痛患者的临床资料,全部患者均在急性期或亚急性期行高刺激率ABR检查,对二者的高刺激率ABR结果进行比较分析。结果梅尼埃病患者纯音测听异常56例(100%,56/56),均为单侧感音神经性聋;高刺激率ABR检查异常7例(12.5%,7/56)。前庭性偏头痛患者纯音测听均正常,高刺激率ABR检查异常27例(73.0%,27/37)。经统计学分析,二者纯音测听及高刺激率ABR检查结果的差异有统计学意义。结论与梅尼埃病相比,前庭性偏头痛患者纯音测听异常较少见,高刺激率ABR异常率较高。在结合患者病史及临床表现的基础上,以上检查可为二者的鉴别诊断提供辅助参考依据。
Objective To analyze the auditory brainstem responses(ABRs) at high stimulus rates in patients with Meniere's disease and vestibular migraine and to provide further evidences for the differential diagnosis of the two diseases. Methods The clinical data of 56 patients with Meniere's disease and 37 patients with vestibular migraine were retrospectively analyzed. All patients received the ABR test at high stimulus rates during the acute or subacute period and the results were compared between the two groups. Results All of 56 patients with Meniere's disease had unilateral sensorineural hearing loss and 7 cases were abnormal in ABRs at high stimulus rates. All of 37 cases with vestibular migraine had normal hearing and 27 cases were abnormal in ABRs at high stimulus rates. The differences in pure- tone audiometric results and ABRs at high stimulus rates were significant between the two groups. Conclusion Compared with patients with Meniere's disease, abnormal pure-tone results is uncommon in patients with vestibular migraine but the abnormal rate of ABRs at high stimulus rates is higher in these patients. Considering the medical history and clinical features, the application of ABRs at high stimulus rates to patients with Meniere's disease and vestibular migraine can provide supplementary evidences for the differential diagnosis of the two diseases.
