摘要 目的:探讨腰椎牵引过程中不同腰椎间盘对应的最优牵引角度,为新型腰椎牵引设备的开发提供依据。方法:基于人体腰椎医用CT图像构建人体全腰椎三维有限元模型,利用Ansys软件,遵循优化设计的方法,定义牵引角度为设计变量,以各节段腰椎间盘伸长量最大值为目标函数,以最大许用应力为状态变量,求得牵引治疗中各节段腰椎间盘的最优牵引角度。结果与结论:腰椎间盘L5/S1、L4/L5、L3/L4、L2/L3、L1/L2对应的最优牵引角度分别为斜向上10°、15°、20°、23°和25°。该方法不仅可以指导腰椎间盘突出症的治疗,而且可以为脊柱牵引治疗系统的开发提供依据。 Aim : To explore the corresponding optimal traction angle of different lumbar intervertebral disc during trac- tion, and to supply basis for the developing of new lumbar traction equipment. Methods: The three-dimensional finite ele- ment model of human lumbar spine segment from L1/L2 to L5/S1 was reconstructed based on medical CT images ; and then through the optimal design method in Ansys,by defining the traction angle as the design variable, maximum elongation of each segment of lumbar as the objective function and the maximum allowable stress as the state variable, it came to a con- clusion that there were some optimal traction angles of each lumbar discs in traction treatment. Results and Conclusion: The simulations reveal that the optimal traction angles for LS/S1, IA/LS, L3/IA, L2/L3 and L1/L2 are oblique upward 10°, 15° ,20° ,23 ° and 25°, respectively ;this method can not only guide the treatment of lumbar disc herniation, but also provide the basis for the developing of new spinal traction treatment system.
