摘要
目的：探讨二维超声与彩色多普勒在甲状腺良恶性结节临床鉴别诊断中的应用价值。方法：选取延安大学附属医院超声诊断科2012年6月-2014年6月收治的84例甲状腺结节患者作为研究对象，术前对患者行二维超声及彩色多普勒超声检查，并将影像学诊断结果与病理诊断结果进行对比分析。结果：84例甲状腺结节患者中，良性结节38例（共68个结节），恶性结节46例（共82个结节）；二维超声诊断良、恶性结节准确率分别为76．47％、82．93％，彩色多普勒超声诊断良、恶性结节准确率分别为79．41％、85．37％，二维超声联合彩色多普勒超声诊断良、恶性结节准确率分别为97．06％、97．56％，二维超声联合彩超诊断准确率显著高于二维超声或彩色多普勒超声（P〈0．01）；甲状腺良恶性结节在回声类型、钙化、边界形态、周边声晕方面具有显著差异（P〈0．01）；甲状腺良、恶性结节血流分布具有统计学差异（P＜0．01）。结论：二维超声联合彩色多普勒超声检查可显著提高甲状腺良恶性结节的诊断准确率，值得临床推广应用。
Objective To investigate the clinical values of two-dimensional ultrasound and color Doppler ultrasound in the differential diagnosis of benign and malignant thyroid nodules. Methods From June 2012 to June 2014, 84 patients with thyroid nodules were selected. Preoperatively, patients were examined by two-dimensional ultrasound and color Doppler ultrasound. And the imaging diagnostic results were compared with surgical and pathological results. Results Among the 84 selected patients, 38 patients developed benign thyroid nodules, with a total of 68 nodules, while 46 patients developed malignant thyroid nodules, with a total of 82 nodules. The accuracy of two-dimensional ultrasound in the diagnosis of benign and malignant thyroid nodules were respectively 76.47%, 82.93%, while that of color Doppler ultrasound were respectively 79.41%, 85.37%. And the accuracy of diagnosis combining two-dimensional ultrasound with color Doppler ultrasound were respectively 97.06%, 97.56%, with statistically significant differences （P〈0.05）. Conclusion The diagnosis combining two- dimensional ultrasound with color Doppler ultrasound improves the accuracy of the clinical diagnosis of benign and malignant thyroid nodules
