摘要
目的探讨拮抗剂方案在体外受精-胚胎移植(IVF-ET)患者中的临床应用和成本效果。方法选取接受IVF-ET治疗且1年内同时采用拮抗剂方案及其它方案的患者110例。按年龄段将其分为1组(<38岁组,60例)及2组(≥38岁组,50例),又将2组按中心常用的促排卵方案分别分为4小组,采用自身对照方法,比较拮抗剂方案与其它促排卵常用方案的临床结局及成本效果比。结果临床妊娠率B1组(33.33%)低于B2组(45.00%),C1组(25.00%)高于C2组(15.38%),D1组(15.38%)低于D2组(25.00%),P<0.05;在<38岁患者中,拮抗剂方案与黄体中期短效长方案及超长方案相比,成本效果比高,P<0.05;在≥38岁患者中,拮抗剂方案成本效果比低于微刺激方案,但高于超长方案,P<0.05。结论在<38岁患者中,拮抗剂方案与黄体中期短效长方案及超长方案相比,为一种相对理想的方案,在≥38岁患者中拮抗剂方案优于超长方案,但略逊于微刺激方案。
Objective To investigate the clinical application and cost effectiveness in the patients undergoing in vitro fertilization- embryo transfer. Methods A retrospective analysis of clinical outcomes and costs was conducted among 110 patients receiving GnRH antagonist protocol and other protocols within one year. According to different ages,patients were divided into group 1（ 8 years old,n = 118） and group 2（≥38 years old,n = 80）. And according to the different protocol,the group1 was divided into four sub-groups,and the group 2 was divided into four groups. Using self controlled method,clinical outcomes and the cost- effective ratio were compared between GnRH antagonist protocol and other protocol. Results From the clinical pregnancy rate,the group B1（33. 33%） was lower than the group B2（45. 00%）,the group C1（25. 00%） was higher than the group C2（15. 38%）,the group D1（15. 38%） was lower than the group D2（25. 00%）（P〈0. 05）; From the economic analysis,GnRH antagonist protocol was better than the other protocols in group1（P〈0. 05）; GnRH antagonist protocol was inferior to the minimal ovarian stimulation protocol,but better than the other protocol in group2（P〈0. 05）. Conclusion GnRH antagonist protocol is an ideal solution in less than 38 years old patients from both clinical outcomes and economic analysis; GnRH antagonist protocol is inferior to the minimal ovarian stimulation protocol,but it is better than the other protocol in more than 38 years old patients.
