摘要 目的研究超声引导下新型泡沫硬化剂治疗体表静脉畸形的疗效评价。方法在超声引导下将泡沫硬化剂注入畸形血管内治疗体表静脉血管畸形患者67例,共71个病灶,比较注射治疗前后病灶的高频超声声像图、多普勒超声和超声造影的变化,多模态超声评估病灶畸形血管的改变,并随访观察其疗效。结果治愈率74.65%(53/71),部分治愈率14.08%(10/71);病情改善率7.04%(5/71);治疗无效率4.23%(3/71)。多模态超声显示治疗后病灶畸形血管数目、血供、DPUS分级及微循环CEUS分级,与术前相比,差异有统计学意义(P<0.001)。其中,注射治疗后出现周围神经损伤1例(1.41%),注射后有局部胀痛9例(12.68%),其他病例未出现并发症。结论 1)超声引导下畸形血管内注射泡沫硬化剂安全、高效、损伤小、治愈率高,易被患者接受。2)多模态超声评估泡沫硬化剂治疗体表静脉畸形疗效的方法简单易行。 Objective To study the effect of new type foam sclerotherapy for the treatment of superficial venous malformation by ultrasound-guided. Methods Inject foam sclerotherapy into the malformed venous of 71 lesions in 67 patients to treat superficial venous malformation by ultrasound-guided. The changes of 71 lesions before and after the treatment were compared by high-resolution ultrasound, doppler ultrasound and contrast-enhanced ultrasound. The changes in density of malformed blood vessels were evaluated and the curative effect was observed. Results The complete recovery rate was 74. 65% (53/71), the partial recovery rate was 14. 08%(10/71), the medical relief rate was 7. 04% (5/71), and the non-response rate was 4. 23% (3/71). Lesions via multimodal sonographic finding before and after treatment were statistically significant(P〈0. 001). The peripheral nerve injury occurred in one case(1.41% )and local pain occurred in 9 cases (l 2.68 %)after injection treatment. All the other cases did not have serious complications. Conclusion The real-time dynamic ultrasound-guided foam sclerotherapy for low- flow superficial vascular malformation is safe, effective, repeatable, with less injury. It has high cure rate and easily accepted by patients. It is worthy of being recommended.
