摘要
目的探讨胆管内支架置入术对老年壶腹周围癌患者肝功能及生活质量的影响。方法选取2010年1月至2013年6年期间就诊的老年壶腹周围癌患者72例,随机分为观察组(34例)和对照组(38例)。对照组患者行胆总管空肠吻合术,观察组患者行内镜下胆管内支架置入术,对两组患者术后的肝功能恢复情况及生活质量进行评价和对比。结果两组患者术前各项肝功能指标相比均无显著差异(P>0.05),术后肝功能各项指标均可见明显降低,但观察组与对照组相比无显著差异(P>0.05);观察组生活质量各项维度得分均高于对照组,其中生理职能(RP)、社会功能(SF)及生理功能(PF)分值,两组相比有明显差异(P<0.05)。结论应用内镜下胆管内支架置入术治疗老年壶腹周围癌患者,操作简便、安全性高且效果显著。
Objectine To explore the influence of bile duct stenting on liver function and quality of life in elderly patients with periampul-lary carcinoma. Methods A total of 72 elderly patients with periampullary carcinoma in this hospital during January 2010 to June 2013 were se-lected and they were randomly divided into observation group（34 cases）and control group（38 cases）. Patients in control group underwent com-mon bile duct and jejunum anastomosis,and patients in observation group underwent endoscopic biliary stenting. The results of liver function test and quality of life in these patients were evaluated and compared between these two groups. Results The indicators of liver function of patients before the operation were compared between these 2 groups,and the difference was not significant（ P ﹥ 0. 05）. The postoperative indexes in these 2 groups were significantly reduced,but the difference was not significant（ P ﹥ 0. 05）. The each dimension scores in patients of observation group were higher than those of patients in control group,and there was significant difference in scores of responsible physiological function（RP）,social function（SF）and physiological function（PF）between these two groups（ P ﹤ 0. 05）. Conclusion Endoscopic biliary stent placement in treat-ment of patients with early periampullary carcinoma has advantages with simple operation,high safety and marked effect.
