摘要
目的观察家庭自测清晨血压对社区老年高血压患者的管理效果。方法 218例社区门诊老年高血压患者采用随机数字表法分为观察组111例和对照组107例。对照组采取社区服务中心门诊常规治疗模式并每月定期健康教育,观察组在常规治疗模式下每周晨起测量清晨血压2次,随访6个月。分析入组时血压以及服用药物情况,比较干预前后诊室血压和清晨血压达标的情况。结果入组时老年高血压患者清晨血压达标率(46.8%)远低于诊室血压达标率(68.3%),差异具有统计学意义(P<0.05)。单用降压药物、2种药物联用、3种以上药物联用比较,高血压患者的血压达标率差异有统计学意义(P<0.05)。6个月后,两组患者收缩压和舒张压均明显低于干预前,观察组收缩压明显低于对照组,差异有统计学意义(P<0.05);观察组清晨血压以及门诊血压达标情况均优于干预前,且达标率均高于对照组,差异有统计学意义(P<0.05)。结论老年高血压患者自测清晨血压,能更有效地控制血压,在社区可行性好,易于推广。
Objective To investigate the effectiveness of the home self-test for the early morning blood pressure in elderly hypertensive patients in the community. Methods Two hundred and eighteen patients with hypertension were divided into two groups by random number method：111 cases in conventional group on additional self-test early morning blood pressure twice a week,the 107 cases in control group were under normal operating mode. All followed up for 6 months. Results The patients enrolled in the early morning BP monitoring in elderly hypertensive with compliance rate（46.8%）,that was much lower than normal BP compliance rate（68.3%）. The blood pressure standard reach in rates among patients who took single drug,in combination with two drugs and three or more combination drugs with hypertensive patients had significantly differences（p〈0.05）. Systolic and diastolic BP in observation group and the control group were significantly lower than the BP levels before the intervention（p〈0.05）. Morning BP and clinic BP were better in self-test group than those parameters before intervention（ p〈0.05）,and the compliance rate was higher. Conclusion The self-test of the morning BP in elderly hypertensive patients show more effective. It is feasible in the community,will worthy of popularization.
