摘要
目的探讨应用实时超声造影(CEUS)技术检查兔VX2肝癌模型的临床价值。方法选取30只新西兰大白兔制成VX2肝癌模型,分别行常规超声检查及经兔耳缘静脉注射超声造影剂行CEUS检查,观察肝癌模型的影像学特点,获得并分析肝癌模型中肿瘤组织的CEUS定量参数的特点,并与肝癌模型的病理切片比较分析。结果兔VX2肝癌结节进行CEUS检查时均表现为典型的“快进快退”增强特点。兔VX2肝癌组织的AS、PI、AUC较周围邻近正常肝组织明显增高(P〈0.05);AT、TTP较周围邻近正常肝组织明显减少(P〈0.05)。CEUS检查对免VX2肝癌结节的总体检出率及对微小肝癌、小肝癌的敏感度和准确度均高于常规超声检查(P〈0.05)。CEUS对直径0.3-0.5cm微小肝癌的检出率(85.71%)高于常规超声(P〈0.05)。与直径〉1.0cm小肝癌比较,直径〈1.0cm微小肝癌的CEUS定量参数AT、TTP明显增加(P〈0.05)。结论CEUS不仅能提供肝癌病灶的微小血管形态结构信息,而且能实时反映其血流灌注情况,有助于小肝癌及微小肝癌的栓出及定性诊断。
Objective To explore the clinical application value of that to check rabbit VX2 liver cancer model using contrast enhanced ultrasound ( CEUS ) . Methods 30 New Zealand white rabbits were made into VX2 liver cancer model .Respectively performed the conventional ultrasound check and CEUS check .Observed the imaging characteristics of liver cancer model .Obtained and analyzed the CEUS quantitative parameters of tumor tissue in the liver cancer model. And compared with the pathological slice of the liver cancer model. Results Rabbit VX2 liver cancer nodules show the typical enhanced features : "fast forward fast rewind" by CEUS. Compared with neighbour normal liver tissue, the AS, PI, AUC of rabbit VX2 liver cancer tissue were increased obviously ( P〈0.05 ) ; the AT, TTP were decreased significantly ( P〈0.05 ) .Compared with conventional ultrasound check, the overall detection rate of rabbit VX2 liver cancer nodules, as well as the sensitivity and accuracy of micro hepato cellular carcinoma ( MHCC ) , small hepato cellular carcinoma ( SHCC ) were higher using CEUS check ( P〈0.05 ) . The detection rate ofCEUS ( 85.71% ) was higher than conventional ultrasound in MHCC with 0.3 - 0.5cm diameter ( P〈0.05 ) .Compared with SHCC with diameter〉l.0 era, the quantitative parameters ofCEUS : AT, TTP significantly increased ( P〈0.05 ) in MHCC with diameter〈1.0 cm. Conclusions CEUS can provide the information of tiny blood vessels structure in liver cancer, can reflect the blood perfusion, help detect and diagnose to MHCC, SHCC.
