摘要
目的观察术前进行数字化假体模板测量对人工全髋关节置换术(THA)假体选择及置入位置准确性的影响。方法选取自2013-07—2014-07在福建医科大学附属泉州第一医院行THA的患者27例,患者随机分为试验组(15例)和对照组(12例),试验组应用数字化假体模板进行术前计划,对照组以传统X线片模板进行术前计划。术后再进行影像学检查验证手术效果。结果试验组术前测量髋臼外展角与术后实际髋臼外展角差值的绝对值为(3.600±3.971)°,对照组为(8.583±6.265)°,试验组髋臼外展角差值的绝对值小于对照组,差异有统计学意义(t=2.080,P<0.05)。试验组术前测量髋臼前倾角为(20.133±3.695)°,术后实际髋臼前倾角为(20.133±13.695)°,差异无统计学意义(t=1.000,P>0.05)。试验组术前计划假体型号与实际使用假体型号一致以及只相差1个型号者13例(86.7%),对照组为7例(58.3%),试验组例数百分比明显高于对照组,但差异无统计学意义(P>0.05)。结论术前数字化假体模板测量可以提高THA假体选择及假体置入的准确性。
Objective To observe the effects of digital templating measure on total hip arthroplasty(THA) prosthesis selection and its position accuracy location before operation. Methods From July 2013 to July 2014, 27 patients with total hip arthroplasty in First Affiliated Hospital of Fujian Medical University, Quanzhou were selected, who were divided into experimental group(n =15) and control group(n=12). For patients in experimental group digital prosthesis templates were used in preoperative planning, and for those in control group traditional X-ray templates were used. Postoperative imaging verified the effect of surgery. Results In experimental group, absolute value of the difference between preoperative and postoperative acetabular abduction angle was (3.600±3.971)°, the control group was (8.583±6.265)°, the absolute value of acetabular abduction angle difference of the experimental group was less than that of the control group, the difference was statistically significant(t = 2.080, P 〈0.05). The preoperative aeetabular anteversion angle of the test group was (20.133±3.695)°, and the postoperative one was (20.133±13.695)°, the difference was not statistically significant (t =1.000, P 〉0.05). The prosthesis models of preoperatitve planning were the same as those in the actual use. The difference of ordy one model was 13 cases (86.7%) in experimental group, 7 cases in the control group(58.3%), the percentage of the number of cases in the experimental group was significantly higher than the control group, but the difference was not statistically significant (P 〉0.05). Conclusion Digital preoperative templating can improve prosthesis selection and placement accuracy in THA surgery.
