内外侧钢板联合治疗在胫骨平台骨折中的临床应用观察 被引量:8

摘要 目的探讨复杂胫骨平台骨折患者采用外侧锁定钢板加内侧支持钢板治疗的临床效果。方法采用回顾性的研究方法,以2012年2月至2013年7月收治的36例胫骨平台复杂骨折患者作为研究组(采用外侧锁定钢板加内侧支持钢板)。2011年4月至2012年1月收治的40例胫骨平台复杂骨折患者作为对照组(采用双支持钢板治疗)。对两组患者的手术情况及术后情况进行统计分析评价。结果研究组和对照组患者在手术时间、术中出血量、术中输血量、骨折愈合时间、住院时间的比较上差异均无统计学意义(P>0.05),研究组的术后完全负重时间14.5±2.1(周)早于对照组的16.1±2.4(周),差异具有统计学意义(P<0.05)。研究组和对照组术后3个月、6个月、末次随访时分别测量TPA、PA角,两组患者间比较,差异均无统计学意义(P>0.05)。末次随访时Rasmusse评分中,疼痛、行走能力,关节稳定性、伸膝关节、膝关节活动度评分比较,差异均无统计学意义(P>0.05);髁部变宽、关节面塌陷、成角畸形评分差异均无统计学意义(P>0.05)。末次随访时研究组患者的膝关节功能优良率达到91.67%,高于对照组的85.00%,但差异无统计学意义(P>0.05)。结论复杂胫骨平台骨折患者采用外侧锁定钢板加内侧支持钢板治疗与采用双支持钢板治疗效果几乎没有差别,都可以取得较好的临床效果,外侧锁定钢板加内侧支持钢板治疗可以缩短患者的完全负重恢复时间。 Objective To investigate the clinical effect of patients with lateral locking plate + inner support plate for treatment of com-plex tibial plateau fractures. Methods This study used the retrospective method,36 patients with tibial plateau of the 2012 February to 2013 July in Department of orthopedics with complicated fractures were enrolled as the study group( using lateral locking plate + inner support plate). Be-tween 2011 April to 2012 January,there were 40 cases of complex tibial plateau fracture patients considered as control group( with double support plate treatment). To analyze and evaluate the operation situation and the technique statistics of two groups of patients. Results The operation time,intraoperative blood loss,blood transfusion volume,fracture healing time,hospitalization time of the study group and the control group pa-tients were compared the difference,which was not statistically significant( P >0. 05). The postoperative full weight-bearing time of study group was 14. 5 ± 2. 1 weeks,while 16. 1 ± 2. 4 weeks in the control group. There were statistically significant differences( P <0. 05). The study group and the control group after 3 months,6 months,at the end of the follow- up TPA,PA respectively to measure angular. There were not sta-tistically significant between two groups( P >0. 05). At the last follow-up,Rasmusse score,pain,walking ability were compared. The differ-ences of the joint stability,and extension of the knee,the knee joint activity score was not statistically significant( P >0. 05). The condylar width,articular surface collapse,angular deformity score differences were not significant( P >0. 05). At the time of the latest follow-up,knee joint function of the patients in the study groups showed the excellent result,the excellent rate was 91. 67%,which was higher than that of the con-trol group of 85%,but the difference was not statistically significant( P >0. 05).
