摘要
目的掌握深圳市乙型肝炎病毒流行状况及基因型分布情况。方法对深圳市居民进行系统抽样,采用ELISA法检测乙肝表面抗原(HBs Ag),分析乙肝病毒感染状况。筛选出252份HBs Ag阳性血清提取DNA,采用荧光PCR方法进行HBV DNA定量检测及基因分型。结果乙肝表面抗原流行情况:共调查3 771人,深圳1岁~59岁人群HBs Ag阳性率为6.68%,低年龄组流行率显著低于高年龄组,差异有统计学意义(P〈0.05)。基因分型选择83份DNA浓度≥1×103copy/ml的阳性标本进行检测,结果 B型检出率为73.5%,C型检出率为20.5%,B+C混合型检出率为1.2%,差异有统计学意义(P〈0.05),无法分型4例(4.9%)。B、C基因型的分布在年龄、性别上差异无统计学意义(P〉0.05)。C基因型感染者HBe Ag、抗-HBe Ag阳性率及HBV DNA水平均高于B基因型感染者,但差异无统计学意义(P〉0.05)。结论深圳地区1岁~59岁人群中HBs Ag携带率低于全国平均水平,乙型肝炎人群中感染的HBV基因以B型为主,C型次之,B+C混合型最少。
Objective To find out the prevalence and the distribution of genotype of hepatitis B virus( HBV) in Shenzhen and provide scientific evidence for the prevention and treatment of hepatitis B. Methods Systematically selected the samples of residents in Shenzhen. Hepatitis B surface antigen( HBs Ag) was detected by ELISA method,and hepatitis B virus infection was analyzed. DNA was extracted from 252 HBs Ag positive serum samples for quantifying HBV viral load and detecting HBV genotypes by real- time PCR method. Results Epidemic situation of hepatitis B surface antigen: 3 771 individuals were recruited.Among 1 to 59 years- old population of Shenzhen,the prevalence of HBs Ag was 6. 68%,the rate of the younger group was significantly lower than that of the elder group and the differences had statistical significance(P〈0. 05). The positive samples of83 specimens with HBV DNA≥1 × 103 copy / ml were detected,among which the detection rate of genotype B was 73. 5%,the detection rate of genotype C was 20. 5%,and the detection rate of mixed genotype B + C was 1. 2%,and the differences had statistical significance(P〈0. 05). There were still 4 cases( 4. 9%) of unclassified genotype. Genotype distribution showed no significant differences in the age or gender among the patients of genotype B and C(P〈0. 05). The rate of positive in HBe Ag,anti- HBe Ag and HBV DNA level of genotype C were higher than those of genotype B,but there was no statistically significant difference(P〈0. 05). Conclusion The prevalence of HBV infection in Shenzhen residents was lower than that of the national average level. The priority genotype of HBV in the area was genotype B,the following was genotype C,and B + C mixed genotype was rare.
