摘要
目的:利用超声技术评价睡眠呼吸暂停(OSAS)患者左室几何构型、颈动脉结构和功能,分析左室几何构型与颈动脉结构和功能的相关性。方法:入选89例OSAS患者,根据患者左室构型的不同分为正常构型组(A组)、向心性重构组(B组)、离心性肥厚组(C组)以及向心性肥厚组(D组)共4组,对比4组心脏参数:左心室舒张末期内径(LVDd)、左心室收缩末期内径(LVSd)、左心射血分数(LVEF)、相对室壁厚度(RWT)、左心室质量指数(LVMI)和颈动脉脉结构和功能参数:内径(D)、内径变化幅度(Dis)、颈动脉局部内中膜厚度(IMT)、顺应性(CC)、扩张性(DC)、脉搏波传导速度(PWV)的差异,分析OSAS患者左室构型参数与颈动脉结构和功能参数的相关性。结果:⑴与A组比较,B组IMT、PWV升高,C组D、IMT、PWV升高,D组D、Dis、IMT、CC、PWV升高,差异均有统计学意义(P<0.05);与B组比较,C组D、IMT、PWV升高,D组D、Dis、IMT、CC、PWV升高,差异均有统计学意义(P<0.05);与C组比较,D组Dis、IMT、CC升高,差异均有统计学意义(P<0.05)。⑵与A组比较,B组RWT、LVDd、LVSd升高,C组LVMI、LVDd、LVSd升高,LVEF降低,D组LVMI、RWT、LVDd、LVSd升高,LVEF降低,差异均有统计学意义(P<0.05);与B组比较,C组LVMI、LVDd升高RWT、LVEF降低,D组LVMI、LVDd、LVSd升高,LVEF著降低,差异均有统计学意义(P<0.05);与C组比较,D组RWT、LVDd、LVSd升高,差异均有统计学意义(P<0.05)。⑶相关分析显示,OSAS患者左心室构型参数LVMI与IMT和PWV呈正相关(P<0.05)。结论:OSAS可引起左室异常重构以及颈动脉结构和功能的损害,其中左室肥厚OSAS患者颈动脉结构和功能损害更为严重,且左室构型与颈动脉结构和功能具有正相关性。
Objective. To explore the evaluation of left ventricular configuration hy ultrasonic examination in patients with obstructive sleep apnea syndrome (OSAS) and the correlation of Carotid artery structure and {unction. Methods: A total of 89 patients with OSAS were selected and divided into the normal geometry (group A), concentric remodeling (group B), eccentric hypertrophy (group C), concentric hypertrophy (group D) depending on the patients with left ventricular configura- tion. Cardiac parameters including left ventricular diastolic diameter (LVDd), left ventricular systolic diameter(LVSd), left ventricular ejection fraction(LVEF), relative wall thickness(RWT), left ventricular mass index(LVMI) and neck affection- ately structural and functional parameters: diameter(D), distension(Dis), intima media thickness(IMT), compliance coeffi- cient(CC), distensibility coefficient (DC), pulse wave velocity (PWV) were compared among four groups. Correlation be- tween cardiac parameters and neck affectionately structural and functional parameters was analyzed. Results: Compared with group A, IMT, PWV in group B were significantly increased, D, IMT, PWV in group C were significantly increased, D, Dis, IMT, CC, PWV in group D were significantly increased (P〈0.05) ~ Compared with group B, D, IMT, PWV in group C were significantly increased, D, Dis, IMT, CC, PWV in group D were significantly increased (P%0.05) ~ Compared with group C, Dis, IMT, CC in group D were significantly increased (P〈0.05).
