食管癌根治术后发生胸腔感染的危险因素分析及护理干预 被引量:7

摘要 目的探讨食管癌根治术后发生胸腔感染的危险因素及护理干预措施。方法选择食管癌患者150例,回顾性调查患者术后发生胸腔感染情况,采用χ2检验,单因素、多因素logistic多元回归对危险因素进行分析。结果 150例食管癌患者术后发生胸腔感染21例(14%),革兰阳性菌检查率52.38%,革兰阴性菌检查率38.10%,真菌检出率9.52%。经单因素分析,术后发生胸腔感染与手术操作时间、肺部病灶蔓延、术后污染、预防用抗生素以及引流管置留时间等因素密切相关,差异有统计学意义(P<0.05);多因素logistic回归方程分析,手术操作时间、胸腔引流管留置时间以及术后污染是导致食管癌术后发生胸腔感染的危险因素。结论医护人员应及时给予相关患者针对性预防保护措施,控制相关危险因素的发生,有效地降低术后感染发生率,提高食管癌根治术的治疗效果。 Objective To investigate the risk factors of chest infection after radical operation for esophageal carcinoma and nursing interventions. Methods A total of 150 patients with esophageal cancer were selected and their chest infection conditions after radical operation for esophageal carcinoma were investigated retrospectively X2 test, univariate, multivariate logistic re- gression were used to analyze risk factors. Results In 150 patients, postoperative abdominal infec- tion occurred in 21 patients with esophageal cancer ( 14 % ). Inspection rate of Gram - positive bac- teria was 52.38 %, the inspection rate of Gram - negative bacteria was 38.10 % and detection rate of fungi was 9.52 %. After univariate analysis, chest infections were closely correlated with opera- tion time, postoperative pulmonary lesions spread, surgery pollution prevention, protective use of antibiotics and drainage tube indwelling time (P ~ 0.05). Multivariate logistic regression showed that operation time, chest tube indwelling time and postoperative pollution were risk factors for esophageal contamination. Conclusion Medical staffs should treat the patients with the targeted preventive measures to control the risk factors, and these measures can effectively reduce the inci- dence rate of postoperative infection and improve the therapeutic effect of esophageal cancer surgery.
