摘要
目的总结小儿脊柱结核手术术中的安全护理经验。方法回顾我院2010年1月至2014年10月进行的小儿脊柱结核手术46例，其中腰椎结核19例，胸椎结核17例，腰骶椎结核6例，胸腰椎结核2例，颈椎结核2例。手术包括后路脊柱内固定椎体病灶清除术18例；后路脊柱内固定＋开胸椎体病灶清除术8例；后路脊柱内固定＋腹膜外椎体病灶清除术14例；侧卧位前路脊柱内固定＋病灶清除术4例；仰卧前路颈椎内固定＋病灶清除术2例。对术中所采取的安全护理措施进行评价。结果46例患儿术中无一例发生护理不良事件。术中未发生皮肤意外损伤、肢体神经功能损伤；变换体位时未发生管路脱出及输液渗漏；未发生术中体温异常；手术苏醒期无呕吐及窒息、坠床发生。结论由于术中采取了一系列安全有效的、科学的、人性化的护理措施，杜绝了护理不良事件的发生，提高了手术护理质量。
Objective To summarize safety nursing care experience during pediatric spinal tuberculosis sur- gery. Methods We analyzed retrospectively the safety nursing care measures taken during internal fixation sur- gery from 46 pediatric spinal tuberculosis cases, including 19 cases of tuberculosis of lumbar spine, 17 cases of tu- berculosis of thoracic vertebra, 6 cases lumbar sacral vertebral tuberculosis, 2 cases of thoracic and lumbar tubercu losis, 2 cases of cervical tuberculosis, admitted in Beijing Chest Hospital, Capital Medical University from Jan. 2010 to Oct. 2014. The operative method included posterior internal fixation of vertebral body lesions removal in 18 cases, posterior internal fixation and open thoracic vertebral lesions debridement in 8 cases, posterior internal fixa- tion and extraperitoneal vertebral body lesions debridement in 14 cases, lateral anterior spinal internal fixation in 4 cases and lesion debridement, supine anterior cervical fixed and lesion debridement in 2 cases. Results There was no nursing adverse event in the operation of 46 cases, such as accidental injury of skin, limb nerve function inju- ry, tube dislocation or infusion leakage occurred when changing posture; intraoperative abnormal temperature; vomi- ting and choking in surgery recovery period, falling bed during surgery. Conclusion A series of safe, effective, scientific, humane care measures can guarantee to prevent the occurrence of adverse events and to improve the quali- ty of surgical nursing care.
