摘要
目的 探讨整体护理对分娩镇痛初产妇和新生儿的影响。方法 选择2014年2~5月在我院行分娩镇痛的200例初产妇,按照产妇对分娩镇痛的自主要求均分为实验组和对照组。对照组待产妇进行分娩镇痛麻醉时无麻醉护士陪同,仅麻醉医师按常规进行操作。实验组在由麻醉医师行分娩镇痛的同时由麻醉护士运用整体护理模式进行护理。观察两组产妇的疼痛分级及镇痛满意度,比较两组的各产程时间、分娩方式、新生儿窒息发生率及产后出血情况。结果 与对照组相比,实验组的疼痛明显减轻（P〈0.05）,镇痛满意度评分显著高于对照组（P〈0.05）;实验组的第1、2产程明显缩短（P〈0.05）,但两组第3产程时间差异无统计学意义（P〉0.05）;实验组的剖宫产率显著低于对照组（P〈0.05）,产后出血量显著少于对照组（P〈0.05）,但两组新生儿窒息率差异无统计学意义（P〉0.05）。结论 整体护理能够缩短分娩镇痛初产妇的产程,降低剖宫产率和产后出血发生率,并明显缓解产妇疼痛,提高镇痛满意度。
Objective To explore the affect of holistic nursing on labor analgesic of primiparas and neonates. Methods200 primiparas in our hospital childbirth analgesia from February to May 2014 were selected and evenly divided into experimental group and control group based on their willing for labor analgesia.The control group was no nurse anesthetist accompanied on anesthetization,and only anesthetist followed the regular procedures.In the experimental group,anesthetist proceeded with labor anesthetization,at the same time,nurse anesthetist applied with holistic nursing.Pain grading,analgesia satisfaction,time of each labor,delivery mode,incidence of neonatal asphyxia and postpartum hemorrhage in the two groups was compared. Results In comparison with the control group,pain in experimental group was greatly alleviated（P〈0.05） and pain satisfaction score was obviously improved（P〈0.05）.The time of first and second labor in experimental group was greatly shortened（P〈0.05）,but time of third labor in two groups was no significant difference（P〉0.05）.The cesarean section rate in the experimental group was significantly lower than that in the control group（P〈0.05）,and the amount of postpartum hemorrhage was significantly less than that in the control group（P〈0.05）,but neonatal asphyxia in two groups was no significant difference（P〈0.05）. Conclusion Holistic nursing can shorten labor of analgesia in primiparae,reduce cesarean section rate and incidence of postpartum hemorrhage,greatly alleviate puerperal pain,and improve analgesia satisfaction.
