摘要
目的:探讨乳腺钼靶X射线摄片联合糖链抗原15-3(CA15-3)、糖链抗原125(CA125)、癌胚抗原(CEA)及细胞角蛋白19片段抗原21-1(CYFRA21-1)4种血清肿瘤标志物对乳腺癌的临床诊断价值。方法:90例经手术病理诊断证实的乳腺结节疾病患者依据诊断结果分为良性肿瘤组(n=32)和乳腺癌组(n=58),所有患者术前均行钼靶X射线摄片检查;另选择同期本院体检的健康妇女56例为对照组,电化学发光免疫分析法检测3组受试者4种血清肿瘤标志物水平,评价单项及联合检测诊断乳腺癌的敏感性、特异度、阳性预测值和阴性预测值。结果:乳腺癌组血清4种肿瘤标志物CA15-3、CA125、CEA和CYFRA21-1均明显高于对照组和良性肿瘤组,差异均有统计学意义(P<0.05);良性肿瘤组和对照组4种肿瘤标志物水平比较,差异无统计学意义(P>0.05);钼靶X射线片诊断乳腺癌的敏感性较高,但特异度较低,而血清肿瘤标志物诊断乳腺癌的敏感性较低,特异度较高,钼靶X射线片联合4种肿瘤标志物诊断乳腺癌的敏感性为89.66%,特异度为78.13%。结论:钼靶X射线摄片联合血清肿瘤标志物CA15-3、CA125、CEA和CYFRA21-1检测可提高乳腺癌检出的敏感度,对于早期诊断治疗有重要临床意义。
Objective:To explore the clinical diagnostic value of Molybdenum Target X-ray combined with serum CA15-3,CA125,CEA and CYFRA21-1tumor markers in the detection of mastocarcinoma.Method:90cases of breast diseases confirmed by surgery pathology were divided into benign tumor group(n=32)and breast cancer group(n=58),all patients underwent preoperative line of molybdenum target X-ray radiography examination.56 cases healthy women visited our hospital for regular check-up were enrolled as control group.Serum tumor marker levels were detected with electrochemiluminescence immunoassay method.Sensitivity,specificity,positive predictive value and negative predictive value of detection of one index alone or in combination were evaluated.Results:CA15-3,CA125,CEA and CYFRA21-1of breast cancer group were significantly higher than that of control group and benign tumor group(all P〈0.05).No significant difference in these levels were found between benign tumor group and the control group(P〉0.05).Molybdenum target X-ray slice showed high sensitivity for diagnosis of breast cancer,but its specificity was low.The sensitivity of detecting serum tumor markers is low for diagnosis of breast cancer,but the specificity was higher.Combing detection of molybdenum target X-ray slice and our tumor markers showed sensitivity of 89.66% and specificity of 78.13%for diagnosis of breast cancer.Conclusions:Combing detection of molybdenum target X-ray slice and four tumor markers including serum CA15-3,CA125,CEA and CYFRA21-1can improve the sensitivity for diagnosis of breast cancer showing high value for early diagnosis of the disease.
