摘要
目的:研究选择性激光融积钴铬合金烤瓷冠修复的效果及对血清和牙龈组织中炎症因子、黏附分子含量的影响。方法:将2013年5月~2014年8月接受钴铬合金烤瓷冠修复的80例患者纳入研究,随机分为两组,观察组进行选择性激光融积钴铬合金烤瓷冠修复,对照组进行铸造钴铬合金烤瓷冠修复。检测两组患者血清和龈沟液中炎症因子、黏附分子的含量。结果:(1)炎症因子:与对照组血清炎症因子含量比较,观察组患者血清中NF-kB、IL-6、IL-8、IL-1β、TNF-α、NO的含量呈降低趋势(均P〈0.05);(2)龈沟液中的黏附分子:与对照组龈沟液中黏附分子含量比较,观察组患者龈沟液中CD11a、CD18、LFA-1、E-selectin、P-selectin的mRNA含量呈降低趋势(均P〈0.05);(3)血清中的黏附分子:与对照组血清中黏附分子含量比较,观察组患者血清中sICAM-1、sVCAM-1含量较低(均P〈0.05)。结论:选择性激光融积钴铬合金烤瓷冠修复有助于缓解牙龈组织的炎症反应,表现为炎症因子和黏附分子生成减少,是冠修复的理想材料。
Objective: To study the effect of selective laser melting deposition cobalt chromium alloy porcelain crown restoration on contents of inflammatory cytokines and adhesion molecules in serum and gingival tissue. Methods: A total of 80 cases who received cobalt chromium alloy porcelain crown restoration in our hospital from May 2013 to August 2014 were en- rolled and randomly divided into two groups. Observation group received selective laser melting deposition cobalt chromium al- loy porcelain crown restoration and control group received casting cobalt chromium alloy porcelain crown restoration. Then contents of inflammatory cytokines and adhesion molecules in serum and gingival crevicular fluid of both groups were detected. Results: (1) Inflammatory cytokines: compared with serum inflammatory cytokine contents of control group, serum NF-kB, IL-6, IL-8, IL-1β, TNF-a and NO contents of observation group were significantly decreased (P〈0.05) ; (2) Adhesion mole-cules in gingival crevicular fluid: compared with adhesion molecule contents in gingival crevicular fluid of control group, mR- NA eontents of CDlla, CD18, LFA-1, E-selectin and P-selectin in gingival crevicular fluid of observation group were signifi- cantly decreased (P〈50.05) ; (3) Adhesion molecules in serum: compared with adhesion molecule contents in serum of control group, sICAM-1 and sVCAM-1 eontents in serum of observation group were significantly lower (P〈0.05). OoneJusion: Selec- tive laser melting deposition cobalt chromium alloy porcelain crown restoration is helpful to relieve inflammatory response of gingival tissue, with expression of decreased generation of inflammatory cytokines and adhesion molecules; it's an ideal material for crown restoration.
