摘要
目的总结老年人食管癌患者围手术期管理及快速康复外科措施应用的效果。方法本组40例老年食管癌患者经术前评估及全身疾病治疗后,分别行翻身三切口颈部吻合食管癌根治术和左胸后外侧切口胸内吻合食管癌根治术,术中术后加强基础管理的同时辅以快速康复外科措施。结果本组40例患者均顺利完成手术,33例顺利康复,7例发生术后并发症,发生比例为17.5%(7/40),死亡2例。结论围手术期管理及快速康复外科措施的应用,有助于手术顺利实施和术后顺利康复,提高手术的疗效。
Objective To summarize the experience of perioperative management of aged patients with esophageal cancer and to explore the application and effect of fast track surgery.Methods 40 cases of aged patients with esophageal cancer,after preoperative evaluation and treatment of systemic disease,respectively underwent turning over three-incision esophageal cancer radical surgery by cervical anastomosis and left chest posterolateral incision intrathoracic anastomotic esophageal cancer radical surgery.During and after the surgery,basic management was strengthened and was assisted by fast track surgery.Results All the 40 cases were successful in the operation.33 cases recovered safely,7 developed postoperative complications （7 /40, 17.5%）,and 2 died.Conclusion Perioperative management and fast track surgery contribute to smooth implementation of the operation and post-operative rehabilitation,as well as the improved efficacy of surgery.
