摘要
目的探析CRRT治疗在危重症合并高钾血症患者中的应用效果及对血钾浓度所造成影响。方法选择解放军一五二中心医院2010年4月—2013年6月期间所收治86例危重症合并高钾血症患者作为研究对象,分为两组。对照组采取内科综合治疗,观察组在对照组基础上采取CRRT治疗。比较两组治疗前后肌酐(Cr)、血尿素氮(BUN)、肌红蛋白(Mb)、C反应蛋白(CRP)等生化指标变化情况、血钾浓度变化情况、住院时间及不良反应发生率。结果两组治疗前各项生化指标均无显著差异(P>0.05),治疗后观察组各指标下降幅度均比对照组更大,组间对比差异明显(P<0.05)。两组治疗前血钾浓度无明显差异(P>0.05),治疗后观察组血钾浓度为(4.6±0.7)mmol/L,低于对照组,下降幅度更大(P<0.05);住院时间为(10.4±1.3)天,低于对照组(P<0.05);不良反应发生率的比较无显著差异。结论应用CRRT对危重病合并高钾血症患者进行治疗可显著改善患者生化指标,促进血钾浓度的恢复,安全性高,应用效果显著,值得推广。
Objective To probe into the treatment of CRRT in the critically ill associated with the application effect of hyperkalemia in patients and the influence of concentration of blood potassium. Methods Totally 86 cases of severe complicated with hyperkalemia in patients treated in our hospital from April 2010 -June 2013 year as research objects, were divided into two groups. Control group took comprehensive treatment of internal medicine department, observation group on the basis of the control group adopted the CRRT treatment. Compare two groups before and after treatment of creatinine (Cr), blood urea nitrogen (BUN), myoglobin (Mb) and C reactive protein (CRP) and other biochemical index changes, the concentration of serum potassium changes, hospitalization time and the incidence of adverse reac- tions. Results There were no significant differences between the various biochemical indicators before treatment of two groups ( P 〉 0.05 ) , after treatment, observation group each index decreased much more than that in the control group, comparison between two groups had obvious difference (P 〈 0. 05 ). The concentration of serum potassium had no signifi- cant difference between two groups before treatment ( P 〉 0. 05 ) , after treatment, observation group serum potassium concentration was (4. 6 ± 0. 7 ) mmo]/L, lower than that of the control group decreased much more (P 〈 0. 05 ) ; hospitalization time was ( 10. 4 ± 1.3 ) days, lower than that of the control group ( P 〈 0. 05 ) ; Incidence of adverse reaction and there was no significant difference. Conclusion The application of CRRT in critical disease complicated with hyperkalemia were treated with biochemical indexes in patients with significantly improved, to promote the concentration of blood potassium recovery, with high security, application effect is significant, is worthy of promotion.
