摘要
目的：分别应用交锁髓内钉和锁定加压钢板治疗远端闭合性胫腓骨骨折的效果及临床价值。方法选择永煤总医院2011年3月至2013年12月共收治72例远端闭合性胫腓骨骨折患者进行研究，分为 A 组和 B 组，分别采取交锁髓内钉和锁定加压钢板治疗方法，比较两组优良率、骨折愈合时间、结骨痂时间及并发症发生率。结果 A 组骨折愈合时间、结骨痂时间分别为（148.4±11.5）d、（38.2±5.1）d，均比 B 组更短，比较差异有统计学意义（P ﹤0.05），优良率为94.6%（35/37），与对照组比较差异未见统计学意义（P ﹥0.05）。A 组出现1例伤口感染、1例骨不连及2例延迟愈合，并发症发生率为10.8%（4/37），显著低于 B 组的22.8%（8/35），比较差异有统计学意义（P ﹤0.05）。结论与锁定加压钢板治疗相比，交锁髓内钉治疗胫腓骨骨折可减轻软组织剥离情况，并发症发生率更低同时骨折愈合、结痂速度更快，预后得到改善，临床应用价值高。
Objective To compare the clinical value and effect of interlocking intramedullary nail and locking compression plate in treatment of closed distal tibial and fibular fracture. Methods Seventy-two cases of closed distal tibial and fibular fracture patients treated in our hospital from March 2011 to December 2013 were selected as research object. They were divided into group A and group B,respectively adopt the interlocking intramedullary nail and locking compression plate,and the good rate,fracture healing time,node callus time and the incidence of complications of the two groups was compared. Results In group A,fracture healing time,node callus time（148. 4 ± 11. 5）d,（38. 2 ± 5. 1）d were shorter than those of group B,and the differences were significant（P ﹤ 0. 05）. The good rate was 94. 6%（35 / 37）,com-pared with control group had no significant difference（P ﹥ 0. 05）. One case of nonunion,one case of bone nonunionand and two cases of delayed healing wound infection in group A,the complication rate was 10. 8%（4 / 37）, which was significantly lower than that of group B （22. 8% ,8 / 35）,the differences was significant（P ﹤ 0. 05）. Conclusions Compared with locking compression plate fixation,fracture of tibia and fibula intramedullary nails treatment can lighten soft tissue stripping,complication rates are lower and the healing scab time faster,the prognosis is improved,with high clinical application value.
