64排螺旋CT仿真结肠镜诊断结直肠占位性病变价值 被引量:8

摘要 目的探讨64排螺旋CT仿真结肠镜(CT virtual colonoscopy,CTVC)在结直肠占位性病变诊断中的价值。方法疑诊结直肠癌及结肠息肉患者135例,均行CTVC、结肠镜活检组织病理检查,计算CTVC诊断结直肠癌及结直肠息肉的敏感性、特异性、阳性预测值、阴性预测值。结果结肠镜活检组织病理检查诊断结肠癌42例,结直肠息肉59例,正常34例;CTVC诊断结直肠癌38例,结肠息肉48例(假阳性4例),诊断结直肠占位性病变的敏感性为85.14%,特异性为88.23%,阳性预测值为95.55%,阴性预测值为66.66%。结论 CTVC检查诊断结直肠癌及结直肠息肉有较高敏感性和特异性,可作为高危人群结直肠占位病变的筛查方法。 Objective To investigate the value of 64-row CT virtual colonoscopy (CTVC) to screening the colorectal occupying lesions. Methods A total of 135 patients with suspected colorectal cancer and polyps underwent both CTVC and colonoscopy biopsy. The sensitivity, specificity, positive predictive value and negative predictive value of CTVC for colorectal cancer and colorectal polyps were calculated. Results Colonoscopy biopsy confirmed 42 cases of colorectal cancer, 59 cases of colorectal polyps and 34 normal cases, while CTVC revealed 38 cases of colorectal cancer, 48 cases of colorectal polyps with 4 case of false positive. The total sensitivity, specificity, positive predictive value and negative predictive value of CTVC for colorectal occupying lesions were 85.14%, 88.23%, 95.55% and 66.66%. Conclusion CTVC can be used as a screening method for colorectal cancer and polyps in high-risk patients, with high sensitivity and specificity.
