黄芪桂枝五物汤加减治疗糖尿病周围神经病变的临床研究 被引量:14

摘要 目的评价黄芪桂枝五物汤加减治疗糖尿病周围神经病变(diabetic peripheral neuropathy,,DPN)的临床疗效。方法纳入78例DNP患者并按随机数字表法分为2组各39例。对照组适当运动、控制饮食并配合降糖药物、甲钴胺和维生素B1;治疗组在此基础上加服黄芪桂枝五物汤,均治疗8周。采用酶比色法检测血清超氧化物歧化酶(superoxide dismutase, SOD)、谷胱甘肽过氧化物酶(glutathione peroxidase, GSH-Px) 和过氧化氢酶活性,采用比色法检测血清丙二醛水平;肌电图/诱发电位仪检测感觉神经传导速度 (sensory nerve conduction velocity, SCV)和运动神经传导速度(motor nerve conduction velocity,MCV)。结果治疗后,治疗组MCV[(43.12±4.1)m/s比(40.27±2.8)m/s,t=3.585]、SCV[(41.51±3.4)m/s比(39.63±3.5)rn/s,t=2.406]均较对照组升高(P〈0.05);SOD[(51.1±6.6)U/ml比(47.6±3.9)U/ml,t=2.851]、GSH-Px[(241.7±36.1)U/ml比(213.9±31.7)U/ml,t=3.614]抗氧化酶活性明显高于对照组(P均〈0.01)。治疗组、对照组总有效率分别为87.2%(34/39)、69.2%(27/39),2组比较差异有统计学意义(X^2=4.333,P=0.033)。结论黄芪桂枝五物汤联合西医常规疗法可改善DNP患者抗氧化能力,控制DNP的发展。 Objective To evaluate the therapeutic effect of Huangqi-Cruizhi-Wuwu decoction in patients with diabetic peripheral neuropathy (DPN). Methods A total of 78 patients with DNP were included and divided into a control group and a treatment group, with 39 in each group. The patients in the control group were administrated with appropriate exercise, control diet, hypoglycemics, mecobalamin and vitamin B1, those in the treatment group were received Huangqi-Guizhi-Wuwu decoction on the basis of the control group. The patients in both groups were Ucated for 8 weeks. The serum activities of superoxide dismutase (SOD), glutathione peroxidase (GPx) and catalase were determined by enzymatic-colorimetric method, and the serum malondialdehyde level was measured using eolorimetric method. Sensory nerve conduction velocity (SCV) and motor nerve conduction velocity (MCV) were measured using an Electromyograph and Evoked Potential Equipment. Results After treatment, MCV (43.12 ± 4.1 m/s vs. 40.27 ± 2.8 m/s; t=3.585, P〈0.05), SCV (41.51 ± 3.4 m/s vs. 39.63 ± 3.5 m/s; t=2.406, P〈0.05) in the treatment group were significantly increased than those in the control group. The serum activities of SOD (51.1 ± 6.6 U/ml vs. 47.6 ± 3.9 U/ml; t=2.851, P〈0.01),GSH-Px (241.7± 36.1 U/m1 vs. 213.9 ± 31.7 U/ml; t= 3.614, P〈0.01) in the treatment group were significantly increased than those in the control group. The total efficiency in the treatment group was significantly higher than that in the control group (87.2% vs. 69.2%; Z2=4.333, P=0.033). Conclusions Huangqi-Guizhi-Wuwu decoction combined with conventional therapy could improve antioxidant capacity and control DPN progress in patients with DPN.
