摘要
目的：探讨带旋髂深血管蒂髂骨瓣移植术治疗股骨颈骨折并股骨干骨折的治疗效果。方法：回顾性分析2008年3月到2014年12月收治的股骨颈骨折患者256例的临床资料,其中股骨颈合并股骨干骨折的患者共有30例,分成观察组和对照组,每组15例,观察组采用带旋髂深血管蒂髂骨瓣移植术治疗,对照组使用切开复位加压空心螺钉内固定术治疗。手术治疗后,分析比较两组患者的平均愈合时间、术后并发症以及Harris髋关节功能评分情况。结果：对所有患者展开20∽70个月随访,平均随访时间43.5个月。观察组术后4∽6个月时间全部愈合,平均愈合时间为（4.2±1.3）个月,髋关节功能恢复优良率93.3%,无患者出现股骨头缺血性坏死。对照组患者的平均愈合时间（6.5±2.9）个月,髋关节功能恢复优良率73.3%,3例患者股骨头出现缺血性坏死。观察组患者的疗效、并发症的发生率以及平均愈合时间都要优于对照组患者。结论：带旋髂深血管蒂髂骨瓣移植术治疗股骨颈骨折并股骨干骨折具有比较好的疗效,值得临床推广使用。
Objective： To study the effect of vascularized iliac bone grafting in the treatment of femoral neck fractures accompanied with femoral shaft fractures. Method： The clinical data of 256 patients with femoral neck fractures hospitalized from March 2008 to December 2014 were retrospectively analyzed. 30 patients with femoral neck fractures accompanied with femoral shaft fractures were divided into the observation group and the control group,15 patients in each group. The observation group was treated with vascularized iliac bone grafting while the control group was treated with internal fixation by using hollow screws,with their effect,incidence of femur head necrosis,mean time of healing,complications and Harris Hip Score after the treatment compared. Results： All the patients were followed up for 20 ∽ 70 months,the average time of which was 43. 5.The patients in the observation group were all cured and no femur head necrosis occurred,with the mean time being 4. 2 ± 1. 3months and the excellent rate of hip joint function being 93. 3%. The patients in the control group were cured in a mean time of 6. 5 ± 2. 9 months,three of whom had ischemic necrosis of the femur head,the excellent rate of hip joint function being 73. %. The clinical effect of the observation group was better than that of the control group; the incidence of the complications of the observation group was lower than that of the control group and the mean healing time of the observation group was shorter than that of the control group. Conclusion： Vascularized iliac bone grafting has a good clinical efficacy on femoral neck fractures accompanied with femoral shaft fractures,which is worth promoting clinically.
